Description

The VA Network 22 Logistics Office seeks competitive offers for a firm, fixed-price requirements contract (Commercial Item FAR Part 12) to provide Conmed Corporation manufactured laparoscopic surgical trays for cost-per-procedure laparoscopic surgical and diagnostic procedures. Services to include weekly, on-site quality insurance inspections of instruments, and replacement of worn or defective instruments at no additional charge to the Government. Supplies and services are to be delivered to VA San Diego Healthcare System, 3350 La Jolla Village Drive, San Diego, CA 92161. Contract is for a one (1) year base period with four (4) additional one-year option periods. Solicitation to be available on/or about 12/28/09 with quotes due on/or about 01/25/10. All requests for the solicitation must be in writing to the Contracting Officer via e-mail at Jan.Smith1@va.gov or faxed to (562) 961-1396. The requester must include the RFQ Number, requester's name, address, telephone number, fax number and e-mail address.
