Description
THIS IS A COMBINED SYNOPOSIS AND SOLICIATION NOTICE. The Veterans Integrated Service Network (VISN) 3 Contracting Office has a requirement for 0.5 FTE Sleep Medicine Consultant (20 hours per week, not to exceed 1000 hours maximum in one Fiscal Year). The base period of performance is anticipated to be middle of January, 2010 through September 30, 2010 plus four (4) 12-month options (through September 30, 2010.) This requirement will be solicited as a 100% Small Business Set Aside. The applicable North American Industrial Classification System (NAICS) code is 621111. The Small Business Administration's size standard for NAICS 621111, Office of Physicians (Except Mental Health Specialists) is $10.0 million. The solicitation number is: VA-243-09-RP-0035. The solicitation and any attachments or subsequent amendments will be available on or about December 04, 2009 on www.fbo.gov. Proposals will be due on or about January 05, 2010 by 3:00p.m E.S.T. Paper copies of the solicitation will not be issued, and telephone and faxed requests for the solicitation will not be honored. Point of contact is Marla Jacobson at Marla.Jacobson2@va.gov. Offerors proposed physician candidate(s) must be board-certified by the American Academy of Sleep Medicine (AASM), possess a current full and unrestricted Medical License in a State, Territory or Commonwealth of the United States or the District of Columbia and must meet all of the qualifications required as listed within the Basis of Award Section of the Request for Proposal document. The contractor physician shall be required to work 20 hours per week for up to 1000 hours per year (maximum). The contractor physician will be responsible to provide consultative services for the comprehensive sleep program to ensure its eligibility for American Academy of Sleep Medicine (AASM) accreditation. Services are to be provided at the Brooklyn Campus of the Department of Veterans Affairs New York Harbor Health Care System (NYHHS) located at 800 Poly Place, Brooklyn, New York 11209. This procurement will be conducted under FAR Part 12, and Acquisition of Commercial Items and FAR Part 15, Contracting by Negotiation. The intent is to make an award for firm fixed price, best value contract for the period of service estimated to be from middle January, 2010 through September 30, 2010 with four (4) one-year options to extend subject to the Government's discretion and subject to availability of funds. Work shall not commence until the awardees' proposed physician candidate completes the credentialing process with NYHHS. Additionally the successful awardees' proposed candidate must complete and submit an application for a security background screening within two weeks of contract award. Proposals must include all required documentation and information outlined in Basis of Award Section of the RFP to be considered responsive. To be eligible for award, the Offeror must be registered in the Central Contractor Registration (CCR) and registration must be valid and up to date. To register in CCR at the website: www.ccr.gov; applicants must have a DUN and Bradstreet number and Tax Identification Number (TIN). Offerors must also be registered in Online Representations and Certifications Application (ORCA), https://orca.bpn.gov/ and registration must be current and up to date.
