STAEMENT OF WORKCOURIER SERVICE The George E. Wahlen VA Salt Lake City Health Care System Pathology and Laboratory Service and the West Valley Community Based Outpatient Clinic require courier service for the transportation of laboratory specimens, supplies, linens, pharmacy products, and mail. 1.Contractor shall provide all necessary labor, equipment, and transportation services each week day for pick up and delivery for the George E, Wahlen VA Salt Lake City Health Care System. The contractor shall pick up commercial goods, supplies, laboratory specimens, linens, pharmacy product, mail and other item as required by the George E. Wahlen VA Salt Lake City Health Care System. The contractor shall provide a driver and vehicle for pick up and delivery. Wahlen VA Salt Lake City Health Care System, 500 Foothill Drive, Salt Lake City, Utah 84148 from Monday to Sunday between the hours of 0800 through 2000 from requested destination. Excluded holidays listed. STAT or Immediate pick-ups for laboratory specimens shall be done within one hour from time of call. Routine pick-ups shall be performed with 2-4 hours from time of call. The contractor shall pick up laboratory specimens from the Western Salt Lake Community Based Outpatient Clinic at 2750 South 5600 West, West Valley City, Utah 84120 and Mental Health Community Based Outpatient Clinic at 2970 South Main Street, Bldg L, Salt Lake City, Utah 84115 at scheduled times, Monday through Friday. The contractor shall pick up pharmacy products from the VASLCHCS at 500 Foothill Drive, Salt Lake City, Utah 84148 and deliver to the Western Salt Lake Community Based Outpatient Clinic at 2750 South 5600 West, West Valley City, Utah 84120 at schedule times, 50 lbs and 25 lbs respectively of pharmacy products Monday through Friday. The contractor shall carry mail to/from the VASLCHCS at 500 Foothill Drive, Salt Lake City, Utah 84148 and Western Salt Lake Community Based Outpatient Clinic at 2750 South 5600 West, West Valley City, Utah 84120 and Mental Health Community Based Outpatient Clinic at 2970 South Main Street, Bldg L, Salt Lake City, Utah 84115 at scheduled times, Monday through Friday. The contractor shall pick up clean linens from the VASLCHCS at 500 Foothill Drive, Salt Lake City, Utah 84148 and deliver to the Western Salt Lake Community Based Outpatient Clinic at 2750 South 5600 West, West Valley City, Utah 84120 and Mental Health Community Based Outpatient Clinic at 2970 South Main Street, Bldg L, Salt Lake City, Utah 84115 at scheduled times, on Fridays. Pharmacy products shall be picked up from the Inpatient Pharmacy on the ground floor in the VASLCHCS and delivered to the Western Salt Lake CBOC pharmacy. Clean linens, and mail shall be picked up from the cage on the First Floor, Building 14, in the hallway by the stairwell door numbered 1AA at the VASLCHCS, 500 Foothill Drive, Salt Lake City, Utah 84148 and delivered to the Western Salt Lake CBOC, 2750 South 5600 West, West Valley City, Utah 84120 room number 1A67 and Mental Health Community Based Outpatient Clinic at 2970 South Main Street, Bldg L Salt Lake City, Utah 84115. Soiled linens will be picked up in room 1A69 and mail picked up in courier mail room from the Western Salt Lake CBOC and Mental Health CBOC delivered to the VASLCHSC to the cage listed above. 4.Documentation Contractor is required to fill out a daily log sheet at time of pick-up or delivery for laboratory specimens. The log sheet requires documentation of time in/out and statement of items being picked up or delivered to be filled. The laboratory employee will verify the specimens transported are received at the correct temperature, delivered, and accounted for by countersigning on the Courier Log Sheet. The laboratory employee will ensure that temperature requirements for laboratory specimens were met during transportation by assessing the received items and document any discrepancies on the log sheet. Issues or problems related to the improper transportation or packaging of laboratory specimens will be directly reported to the COTR. The process will be evaluated and recommendations made for corrective action. 5.Services Delivery Summary The contractor service requirements are summarized into performance objective that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. PERFORMANCE OBJECTIVEPERFORMANCE OBJECTIVE The contractor shall pick up and deliver commercial goods, supplies, And other items as required by the VA Salt Lake City Health Care System.(2) Valid deficiencies per quarter The contractor shall pick up and deliver laboratory specimens in accordance to temperature requirements and all specimens shall be properly packaged and labeled to indicate the general nature of the materials being transported.(2) Substantial deficiencies per quarter The contractor shall ensure the confidentially of all patient information being transported and will be held liable in the event of breach of confidentiality.No defects allowed 6.Performance The contractor will be evaluated on their ability to comply with the performance objectives to fulfill the contract successfully. A Contracting Officer's Technical Representative (COTR) shall be appointed by the government to monitor and evaluate the contractor and ensure all performance objectives are met through review of the log sheet, review of the contractor's quality control program, and receipt of complaints from department personnel. Issues or problems related to the improper transportation or packaging of laboratory specimens, supplies, linens, pharmacy products, or mail will be directly reported to the COTR. Contractor's courier processes will be evaluated and recommendations made for corrective action. Contractor will be requested to submit a plan of corrective action to the COTR. Failure or deficiency of courier service for pick-up will result in withholding of payment for missed service and reimbursement of cost for alternate courier service for the particular pick-up. Damages to the government will be assessed after 2 deficiencies per quarter. Documentation of certified training for all personnel involved in the packaging and shipping of infectious material will be provided to the George E. Wahlen VA Salt Lake City Health Care System before the contractor begins working on the contract. Is there documented certified training of all personnel involved in the packaging and shipping of infectious materials? NOTE: Federal and international regulations mandate the proper packaging and transportation of infectious substances, also termed "etiologic agents." Infectious materials are now classified into two categories, category A (referred to as "infectious substances") and category B (referred to as "biologic substances, Category B"). Category A substances are capable of causing permanent disability or life-threatening or fatal disease in otherwise healthy humans or animals when exposure occurs. Category B substances are not in a form generally capable of causing permanent disability or life-threatening or fatal disease in otherwise healthy humans or animals when exposure occurs. Several examples of category A substances include organisms such as Brucella abortus (cultures only), Ebola virus, or Mycobacterium tuberculosis (cultures only). Most laboratory specimens fall under category B. Refer to the references below for further information. Specific requirements are set forth by the U.S. Public Health Service, the U.S. International Air Transport Association (IATA), and the U.S. Department of Transportation and the U.S. Postal Service. These apply to domestic transportation by land, air or sea, and to international air transportation. All personnel who package specimens for shipment must satisfactorily complete certified training in these requirements. Certified training is required every 2 years. The laboratory may send personnel to courses for certified training, or may obtain materials to train its personnel in-house. Resources for certified training are available from many sources, including state health departments, vendors of shipping materials, and the CDC National Laboratory Training Network (NLTN). I had spoken to the driver and asked what was to be used and he did not have a response other than he would hand carry any specimens. All specimens shall be properly packaged and labeled to indicate the general nature of the materials being transported. All specimen packages shall be packed in a container secured to the vehicle body to prevent movement during transport. Laboratory specimens are to be transported in accordance to temperature requirements. Room temperature specimens will be transported inside a cooler and must remain at a temperature from 22C to 25C during transport. Refrigerated specimens will be transported inside a cooler packed with regular ice and must remain at a temperature from 2C to 10C during transport. Frozen specimens will be transported inside a cooler packed with dry ice and must remain at a temperature for -15C to -20C during transport. 2. ID BADGE REQUIREMENTS As per Information Letter 90-01-6, Contractor Personnel Security Requirements. All contractors, subcontractors and their employees will be required to provide two (2) forms of personal identification (list of acceptable ID is attached), fill out a Declaration of Federal Employment OF 306, be fingerprinted and badge prior to starting work at the VA Salt Lake City Health Care System. The prime contractor shall submit a list of every individual that will be working on the VA campus and a copy of their Employment Eligibility Verification Form I-9 If the individual will have access to VA patient records or computer system, in addition the steps above, they will be required to fill out a Questionnaire for Non-Sensitive Position, Standard Form 85. This form is very extensive and the individual will require approximately 1-2 hours for completion. This form can be done PRIOR to reporting to Building 4. There are two mandatory training classes; Providing a Safe and Secure Environment for Health Care and Information Security and HIPAA (Privacy Act), that will be required for each individual, also. These can be accessed at www.uchep.com Please allow approximately 1 hour to complete the two modules. You will need to complete these two courses and print out the certificates of completion PRIOR to coming to the VA for the ID Badge. Each individual will be required to clear the VA and turn in their badge once the job is completed or they are no longer working at the VA. Final payment will be held until all paperwork and badges have been turned in. If a badge is lost, there will be a charge of $50 for each badge that will be deducted form the final payment. 3.PATIENT CONFIDENTIALITY: The contractor shall ensure the confidentiality of all patient information being transported and will be held liable in the event of breach of confidentiality. The contractor shall comply with the provisions of the Federal Privacy Act of 1974 (Public Law 93-579), the comprehensive Alcohol Abuse and Alcoholism Prevention, treatment and Rehabilitation Act of 1972 (Public Law 93-282), and the Drug Abuse Office treatment Act of 1972 (Public Law 93-255), and any other statutes regarding confidentiality of patient information. 4.PERSONNEL: The contractor is responsible for providing a fully qualified on-site Contract Manager. The Contract Manager shall be responsible for management and coordination of the contract and shall be the point of contact with all government representatives. The contractor shall notify the contracting officer in writing prior to contract start date the names of the Contract Manager and an alternate Contract Manager. The Contract Manager shall be available for the government to reach during normal business hours. The Contract Manager shall respond within one business day from the time of notification. During non-business hours, the Contract Manager shall respond within one business day.
