- Diagram and Buy American Act Certificate This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA7000-10-T-0011, in accordance with FAR Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-37, 13 November 2009. The North American Industry Classification System (NAICS) code is 337215 and the business size standard is 500 employees. This proposed firm-fixed-price acquisition is a 100% Total Small Business set-aside. All responsible sources may submit a response, which if received timely, will be considered by the 10th Contracting Squadron. The Government does not intend to pay for information provided under this synopsis. The required items (CLINS 0001 - 0002) are: CLIN ITEM DESCRIPTION QUANTITY UNIT UNIT PRICE TOTAL PRICE 0001 Electric Mobile Shelving 1 Each This system must meet the following physical, performance and functional characteristics: 1) all dimensions provided by the Government are approximate, the contractor is responsible for performing any necessary measurements and determining the exact sizes required; 2) color: shelves must be charcoal (dark gray or closest available color) and the end of each shelving unit must be Air Force Blue (or closest available color); 3) each shelf must be able to support a total of 500 lbs; 4) the required materials for the shelving units are metal with vinyl or laminate siding; 5) each shelving unit must have electric controls on both ends; 6) each shelving unit must have label holders on both ends; 7) the entire system requires six double sided mobile units (carriages), one double sided stationary unit and one single sided stationary unit; 8) the approximate available space dimensions are: 30'L x 25'W x 8.5'H; 9) the double faced units must be approximately 3'W; 10) double faced units must not have a center divider; 11) the single faced unit must be approximately 1.5'W; 12) walk space between units must be approximately 4'W; 13) total system height must be approximately 7.5'H; 14) total system length must be approximately 27.5'L (to include space between carriages); 15) total system width must be approximately 17.5'W; 16) the unit must sit 3.5' from the side wall to allow adequate walk space; 17) each unit must have five horizontal shelves approximately 3.5 ft W and three vertical shelves with an approximate height of 2.3 ft each, with a total of 15 shelves on each side; 18) shelves must be vertically adjustable; 19) removal and disposal of the existing electric mobile shelving system must be included in your quote; 20) the contractor will be required to dispose of the existing unit off-base; 21) in order to limit patient care impact, the removal of the existing unit and installation of the new unit must take no longer than 5 days and must be performed consecutively during 2 weekend days and 3 week days; 22) use of a subfloor or ramp system installed over the current ground rail system will not be allowed; 23) in order to reduce costs and installation time vendors must use the ground rails currently in place; 24) installation of an additional ground rail(s) is acceptable as long as the project can be completed in the allowed time (5 days); 25) the ground rail located in the aisle, that will not be used, must be removed or filled in such that it will not protrude above the surface; 26) the system must include the manufacturers standard safety features; 27) the contractor will be required to wire the entire system; and 28) a minimum of a one-year warranty which covers parts, labor and manfucturer defects must be included. Installation will take place at, 4102 Pinion Dr., USAF Academy, CO 80840, FOB Point: Destination. Delivery and installation is required within 8 weeks after receipt of order. This product must comply with the Buy American Act (BAA) (100% manufactured in the USA). If an item that is not BAA compliant is offered, the item description, CLIN number, and place of product manufacture must be annoted on the BAA certificate. The following Federal Acquisition Regulation (FAR) clauses apply to this combined synopsis/solicitation and offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors--Commercial Items, addenda applies: The contractor shall submit their quote on company letterhead to include the following: solicitation number, the time specified in the solicitation for receipt of offers, contact name, address, telephone number of the offeror, unit price (shipping cost included in unit price), total price, any discount terms, cage code, DUNS number, tax identification number, size of business, warranty information and a statement that your company is in compliance with the provisions at FAR 52.212-3 Online Representations and Certification Application (ORCA) (http://orca.bpn.gov/publicsearch.aspx) and DFARS 252.225-7000 (http://farsite.hill.af.mil/), Buy American Act-Balance of Payments Program Certificate, which must be signed and returned with the quotation. In addition, any acknowledgement of solicitation amendments must be included. In accordance with FAR 52.212-4, contractors must be registered in the Central Contractor Registration to conduct business with the Department of Defense. No purchase order can be awarded to any company without this registration which can be completed online at www.ccr.gov. Quotes must be received NLT 1600 Mountain Time, 11 December 2009 at 10 CONS/LGCB, Attn: Matt McLean, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315. Offers received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. The provisions at FAR 52.212-2, Evaluation--Commercial Items apply to this acquisition. The Government will award a contract resulting from this solicitation to the responsive responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: award will be made on the basis of determining lowest evaluated price that is technically acceptable which meets or exceeds the requirements. Once an offer has been determined to be technically acceptable then the offeror with the lowest evaluated price will be awarded the contract. Offerors must provide sufficient information such as drawings, descriptions or product literature that will show their product meets all of the characteristics stated above. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be acceptable, and therefore will not be considered. If an offer is not technically acceptable it will not be considered. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
