Description
Statement of WorkSheridan VAMC Mobile MR Unit Philips 1.5T Achieva MR (Mobile) and Associated Coach Service Contract I.Overview The Contractor for the Sheridan VA Medical Center in Sheridan, Wyoming shall provide two options regarding the Sheridan VAMC Mobile MR Unit, consisting of the Philips 1.5T Achieva MR and associated coach. The first option is to provide a Full Service Turn-Key Solution Contract to include all equipment for the Philips 1.5T Achieva MR and the associated coach for a stationary (non-mobile) unit. The second option is to provide a Full Service Contract for the MR system to include all parts, labor, cryogens, and chiller, and provide a Transportation Contract to include a driver, a tractor to transport MR unit, one round trip transfer per week of approximately 300 miles inclusive, and managerial oversight of all maintenance and services on the coach for a mobile MR unit. The Contractor will include the following for both, Option 1 and Option 2, of the contracts: (1)Contractor must be an authorized service representative for the Philips 1.5T Achieva MR (Mobile) unit and the associated coach. (2)Contractor is responsible for the expense of all parts, excluding consumables, for the Philips 1.5T Achieva MR (Mobile) unit. Contractor is responsible for all priority parts delivery expenses. (3)Contractor is responsible to provide a Magnet Maintenance Package (MMP) to include cryogens, magnet insurance, and magnet refrigeration services. Surface coil coverage must also be provided. (4)Contractor is responsible for all labor and travel expenses to provide all services covered, including preventive maintenance, emergency repair, maintenance credit for equipment malfunction, and replacement of MR equipment and parts, during the term of the agreement. (5)Contractor is responsible for all Operating System Software updates and Hardware Reliability updates. Contractor may be able to provide remote diagnostic capabilities. (6)Preventive Maintenance (PM) is to be scheduled at the convenience of the Sheridan VA Medical Center, and will consist of thorough and comprehensive electrical and mechanical checks to identify needed service or maintenance, minor repairs, and calibrations. PMs are performed according to the OEM recommendations (unless otherwise stated), starting four (4) months following the start of the contract agreement. (7)Emergency repairs shall be performed after one (1) initial telephone notification that the equipment is inoperative. The contractor shall provide the Government with a designated point of contact and telephone number and shall make arrangements to enable the maintenance representative to receive such notification. (8)The contractor shall be on-site at the Sheridan VA Medical Center in Sheridan, Wyoming to perform emergency repair services during the week within twenty-four (24) hours after receipt of initial telephone notification of a malfunction. Contractor shall provide weekday on-site technical representative maintenance, troubleshooting, and repair coverage from 8:00am - 5:00pm mst., Monday thru Friday (excluding holidays). Preferred rates for labor and travel outside the principle period of coverage will be required. Federal Holidays observed by the Sheridan VA Medical Center are: New Year's DayMartin Luther King DayPresident's Day Memorial DayIndependence DayLabor Day Columbus DayVeteran's DayThanksgiving Day Christmas Day (9)Contractor shall provide telephone technical support service to assist in troubleshooting 24 hour per day, seven days per week, including Federal holidays. (10)The contractor shall furnish a malfunction incident report to the institution upon completion of each repair call. The report shall include, at a minimum, the following: a)Date and time notified; b)Date and time of arrival; c)Serial number, type, and model number(s) of equipment; d)Time spent for repair; e)Description of malfunction and repair; and f)Proof of repair that includes documentation of a sample run with acceptable performance. (11)During the term of the Service Contract, should the repair record of any individual piece of equipment covered reflect a downtime of 4% or greater of the normal working days in one calendar month, a determination will be made by the designated representative of the Government at the Sheridan VA Medical Center to replace the initial equipment with new equipment. The responsibility of maintaining the equipment furnished in good condition in accordance to the manufacturer's instruction, shall be solely that of the contractor. All equipment covered in this Service Contract shall maintain an uptime of 96% in each month of the term of the agreement. (12)The contractor shall treat each notification for an emergency/repair service call from the Government as a separate and new service call. (13)A full system inspection shall be performed prior to execution of the agreement. Any service determined necessary from the inspection must be completed prior to execution of the agreement upon which a system site number will be assigned. (14)The service contract agreement will run during the term of the contract for one (1) year, followed by four (4) option years, with the ability to cancel with 60 days notice by the Government. II.Additional Statements of Work Specific for Option 1 and Option 2. A.Option 1: Stationary MR Unit The Contractor for the Sheridan VA Medical Center in Sheridan, Wyoming shall provide a Full Service Turn-Key Solution Contract to include all equipment for the Philips 1.5T Achieva MR and the associated coach (i.e. generator, lift, doors, chiller, AC / heating systems) as outlined above. The Service Contract to include preventive maintenance, emergency repair, maintenance credit for equipment malfunction, replacement of all MR and associated coach equipment and parts excluding consumables, labor, and travel for the Philips 1.5T Achieva MR and the associated coach. The Full Service Turn-Key Solution Contract will include all requirements as listed above and the following: (1)Contractor is responsible for all requirements as listed above under Section I for both the Philips 1.5T Achieva MR Unit and the associated coach. This contract is to include all parts, labor, maintenance, and expenses for both the MR unit and the associated coach, excluding consumbles. (2)Under this option, the Mobile MR unit will be parked as a stationary unit at the Sheridan VA Medical Center in Sheridan, Wyoming. The service contract under option 1 must be negotiable for addendums or cancellation, i.e. if the scanner is to go mobile and a travel contract, or clause, is needed. The Government must give the contractor 60 days notice of said changes to cancel contract or start agreement under option 2. B.Option 2: Mobile MR Unit The Contractor for the Sheridan VA Medical Center in Sheridan, Wyoming shall provide a Full Service Contract for the Philips 1.5T Achieva MR system to include all parts, labor, cryogens, and chiller. The Contractor shall also provide a Transportation Contract for the Sheridan VAMC Mobile MR unit to include a driver, a tractor to transport MR unit, one round trip transfer per week of approximately 300 miles inclusive, and managerial oversight of all maintenance and services on the coach for the mobile MR unit. The Full Service Contract and Transportation Contract will include the following: (1)Contractor will provide all requirements as outlined above under Section I. (2)Contractor will provide a tractor and a driver to transport the MR Coach one round trip per week of approximately 300 miles inclusive. a)During transport, liability insurance on the coach and MRI contents to include replacement (new) value. b)Provide all supplies associated with the movement, management, and maintenance of the coach. c)Responsible to maintain the coach in accordance with DOT regulations and as noted herein. d)Setting up the MRI coach at each site by leveling the coach, connecting the MRI coach to VA provided electricity, data, and telephone lines. This must be completed and tested for functioning by the start of normal business hours. VA will provide connections and system testing protocols and contact personnel for test failures. MRI coach shall be generator ready (including a full fuel tank) at all times to manage an electrical outage at the site. e)When roads are closed by competent authority, the contractor is not responsible to perform until the roads are open and the Government is not liable for any additional costs incurred by the contractor as a result of the road closures. Depending on the weather conditions, the MRI coach transport between sites may be delayed and additional patient scheduling may occur at current locations; and/or for road closure days lost, alternative days will be scheduled to make up the loss with mutual agreement between the Contractor and the Government. f)Contractor shall meet all safety requirements such as posting MRI hazardous signs around the mobile unit and providing protection for the electrical power cable (under a metal cover if it crosses a sidewalk), etc., to assure patient, employee, contractor, traffic, and pedestrian safety. g)The driver shall be appropriately licensed to operate the tractor and will supply documents on request in accordance will all federal, state and local statutes. h)All licenses, inspections, adjustments, calibrations, and certifications must be current at all times. VA shall be provided copies of these documents within ten days of award and within ten days of inspection/adjustments/calibrations/change of personnel after award. i)For any loss in MRI scanning time at any site due to Contractor unable to perform its duties, the time lost will be deducted from contractor reimbursement based on a calculated hourly rate for the contractor's services (weekly rate quoted divided by 40 hour week times number of hours lost). (3)Contractor will provide managerial oversight of all coach-related repairs and routine maintenance. (4)Under this part of the contract, the Sheridan Mobile MR unit will be mobile. The service contract under option 2 must be negotiable for addendums or cancellation, i.e. if the scanner is to parked stationary. The Government must give the contractor 60 days notice of said changes to cancel option 2 contract or start agreement under option 1.
