Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Dentist to provide services at the Fort Totten PHS, IHS Healthcare Center in Fort Totten, North Dakota. The period of performance will be from Date of Award through September 30, 2010. The Contractor shall perform according to the Scope of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-010, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-10-010 and provide a postal mailing address, email address, and fax number where the RFQ package can be sent. This is a small business set-aside.
