Description
THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a requirements contract. The Government will use responses to this notice to make to appropriate acquisition decisions. The contractor shall furnish on site Gastroenterology Services to include the following: a. Percutaneous gastrostomy at VAMC, b. Gastrointestinal consultations, c. Upper GI endoscopy with and without biopsy, d. Upper GI endoscopy with sclerotherapy, e. Colonoscopy with biopsy, f. Flexible sigmoidoscopy with and without biopsy, g. Endoscopy with esophageal dilation, h. Liver biopsy, i. UGI endocscopy with esophageal variceal banding. All work shall be performed at the Cheyenne VAMC in Cheyenne Wyoming. The VA Medical center will provide all related supplies needed to perform the required services. In addition, the contractor shall be available for "first call" for emergency consultation and emergency procedures. Furthermore, the contractor shall respond by phone within one hour of receiving a call with follow up on site as determined by VA. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist Matt Ake no later than 3:00 p.m. (Mountain Time), 4 December, 2009. Solicitation VA-259-10-RP-0032 - CHEYENNE GASTROENTEROLOGY SERVICE has been assigned and should be referenced on all correspondence regarding this announcement. NAICS Code 621111 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of $10.0M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
