- OPTOMETRY SERVICES Contractor will provide eye exams/refractions, contact lens and visual care assistance to eligible veterans. Eligibility will be determined by the Prosthetic Service, VA Medical Center, Grand Junction, CO. Contractor must contact all eligible veterans and schedule an appointment within 10 workdays of receiving the eligibility referral. If contact is not made within 10 working days, the name of the veteran and the reason the appointment was not made will be forwarded, in writing, to the Prosthetic Service, VAMC, Grand Junction, CO., within two workdays following the 10 day notice requirement. Contractor must set up an appointment and see the veteran within 20 working days from the date of the consult. If veteran is not seen within the 20 working days, then the name of the veteran and the reason why veteran cannot be seen within the 20 working days must be forwarded in writing to the Prosthetics Service, VAMC, Grand Junction, CO., within 5 working days. A written copy of the eye refraction will be provided to each veteran at the conclusion of the eye refraction. Eye refractions or eye examinations performed by the contractor without prior approval, by the Prosthetic Service, VAMC, Grand Junction, will be at no cost/charge to the VA. Contractor will provide adequate space to display, publically, current VA contracted eyeglass frames. Contractor will provide assistance in selecting the appropriate frame, pupil and frame measurements. Contractor will provide patient education and information to assist the veteran with decisions on type and availability of lens, options for eyewear and other visual care needs. Contractor will utilize/complete the VA Form 10-2914, with the veterans name, last four numbers of the SSN, the appropriate prescription, and date of the examination/refraction, frame selection and measurements. Additionally, this form is used to record special or unusual requirements, and when a veteran request the completed glasses are mailed directly to their home address. (VAMC, Prosthetic Service will mail all glasses to veterans). The optometrist will sign VA Form 10-2914. The completed VA Form 10-2914 will be faxed to the Prosthetic Service, VAMC, Grand Junction, 970-244-7724 within two workdays following the completion of the refraction/exam. Contractor must provide written medical need and justification for specific visual needs, such as, safety lens, sunglasses, (not transition or photogray), a second pair of glasses, prismatic lenses and other ophthalmic aids. This written documentation will be placed in the veterans electronic medical record. The completed VA Form 10-2914 will be used by the Prosthetic Service to order eyeglasses from the national VA contract. Completed eyeglasses will be delivered back to the contractor, unless mailed directly to the veteran by the VAMC, Prosthetic Service. The contractor upon receipt of the glasses will verify the eyeglasses are made within the American National Standards Institute (ANSI) Z80 standards of manufacturing tolerances. An optician, optometrist or ophthalmologist will examine the eyeglasses for damage or defects ensure the lenses are correct according to the written prescription. The contractor will contact the veteran within 3 working days after receiving the glasses to schedule an appointment for adjustment and/or fitting. If contractor cannot get a hold of the veteran after 3 attempts then contractor must notify the VAMC Prosthetic Service in writing the dates and times of the attempts. When the adjusting and /or fitting optician, optometrist or ophthalmologist determines that the eyeglasses do not comply with the written prescription, a determination must be established if the eyeglasses were manufactured incorrectly. In this case the VA Form 10-2914 will be annotated and the VA Form returned to the VAMC Prosthetic Service, along with the glasses for corrections. In cases were the eyeglasses are manufactured correctly to the written prescription, but the veteran can not see clearly, the contractor will re-exam the veteran (at no additional cost to the veteran or VA) and determine the needed changes. VA Form 10-2914 will be annotated, i.e. Script (Rx) change or Dr. Change) and the form sent back to the Prosthetic Service along with the glasses. Prosthetic Service will reorder the glasses. Veterans may request glasses be mailed directly to them. Contractor will make this notation on the VA Form 10-2914, block # 15. (Street address, (no PO Box) City, State and zip code). VAMC Prosthetic Service will mail all glasses to veterans. Contractors will use/conform to the VA computerized patient records system (CPRS) to record all appropriate documentation for the continuation of patient care and record all appropriate eye exams/refractions and recommendations for veterans visual care. Contractor will ensure that all eye refractions, eye examinations, and medical notes pertaining to the veterans eye care are recorded in CPRS. Contractor will meet the requirements/training to obtain VPN/remote access. VA will assist in training in CRPS and provide guidance for obtaining VPN/remote access. Contractor will provide outside referral for visual needs outside the provisions of this contract. Eye Examinations The contractor shall provide comprehensive eye examinations, refractions, contact lens and written prescriptions to those eligible veterans as determined by VAMC Prosthetic Service, Grand Junction, CO. The contractor and staff must possess all current licenses, permits, accreditations and certificates as required by state and federal laws. Optometrist who possess a full and unrestricted license to practice in the state where services are performed, will have to perform these services. No certified technician will be able to perform the eye examinations. Has to be a license Optometrist to perform these eye examinations. Components of this examination 1)Patient demographics (age, race, sex) 2)Chief complaint with appropriate history 3)Patient ocular history (including surgery, laser, trauma) 4)Patient medical history (including diabetes, hypertension, pulmonary disorders, cardiac problems, stroke and any other pertinent history) 5)Family ocular history (including glaucoma and blindness) 6)Ocular medications (including medication, dosage, schedule and time of last dose) 7)Pertinent systemic medications (including prednisone) 8)Medical allergies 9)Distance visual acuity (with current rx if available) and pinhole 10)Neutralization of current rx 11)Manifest refraction for distance and near with be visual acuity for each eye at distance and for both eyes at near 12)Pupils (including direct, consensual response and presence or absence of afferent papillary defect) 13)Rx given if different from manifest refraction 14)Motility of extraocular muscles 15)Confrontation visual fields 16)External exam (as indicated) 17)Slit lamp exam (including lids/lashes, conjunctiva, cornea, anterior chamber, iris, lens) 18)Applanation tonometry (including time and date) 19)Gonioscopy if Van Herrick estimation of angles<grade II 20)Dilated fundus exam (including complete description of cup/disc ratio, presence of notching or pallor, documentation of macula, posterior pole, vessels, periphery and vitreous) Measuring papillary distance, near and far Sizing veteran for eyeglass frames Taking of multi focal measurements Verifying prescriptions in optical lenses Providing quality control through inspection of prescription eyeglasses Adjusting finished eyeglasses to recipients Accomplishing repairs to eyeglasses furnished to the Veteran Act as a consultant to Eye Clinics and Prosthetic Section personnel in all matters pertaining to new developments in the field of prescription lenses. Complete paperwork/invoicing as specified, all services, to include adjustments and repairs at no charge to the government. The invoice shall include vendor information per FAR 52.214 (g), Veteran complete name and social security number, date of services, type(s) of service by description and HCPC, cost of each service, and total cost for all services provided on date of service(s).
