Description
Combined Synopsis/Solicitation with quote submission instructions and evaluation factors. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, and Subpart 13.106 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The title for this request for quotation, "On-line survey program" shall be referenced on any quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. The U.S. Environmental Protection Agency's (EPA) Office of Human Resources (OHR) is soliciting potential offerors, in order to competitively award a fixed-price Purchase Order, as a total small business set-aside. Under this FAR Part 13, commercial acquisition, the contractor shall provide on-line questionnaire/survey services to conduct National Training Needs Assessment and training and development program evaluations. The primary purpose of this requirement is to procure on-line survey capability to send, retrieve, and analyze data collected from EPA employees regarding their impressions of EPA training programs. OHR will use the on-line survey service to help manage the multitude of training program evaluations in a very succinct and effective manner. Any small business concern under NAICS 541519 believing itself to be capable of meeting EPA's requirement, as stated within the Statement of Work, may submit a quote, which, if received by the response date of this announcement shall be considered. Prospective sources are reminded that an award can only be made to a Contractor who is registered in the Central Contractor Registration (CCR) database, in accordance with FAR 4.11. A formatted copy of the Statement of Work, including the evaluation factors and quote submission instructions are attached to this notice.
