- tec evaluation criteria HHA SOW Home Health/Hospice/ESRD Consulting Services Reference# 221-0-306601 This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services (CMS) intends to award using Simplified Acquisition Procedures, a fixed priced purchase order for Consulting Services. A purchase order award is anticipated to be made within (5) business days after the response due date of this notice. The period of performance will be award through September 30, 2010. The Statement of Work/Deliverables and Technical Evaluation Criteria are attached. The North American Industry Classification System Code (NAICS) is 541990. The purpose of this procurement is to acquire the consulting services of someone with expertise and extensive experience with CMS home health and hospice regulations and policy, previous State agency surveyor experience and/or State OASIS education/automation coordinator familiarity. An individual familiar with the operations and certification responsibilities of State agencies and is knowledgeable of the Medicare regulations and survey processes the HHA and hospices. This procurement will enable the necessary revisions to our survey process, program manuals, policies, guidelines and training materials. This Request for Quotes (RFQ) is being set aside for small business concerns. Both the technical and business quotation shall be submitted in hard copy (2) two originals to: Centers for Medicare and Medicaid Services, 7500 Security Blvd., M/S C2-21-15, Baltimore, MD 21244-1850, no later than 11:00 am on November 10, 2009. In addition, an electronic format shall be submitted to Lucille.Lee@cms.hhs.gov and to project officer Kim.Roche@cms.hhs.gov. Responses shall be no more than five pages and shall include the following: 1) Contractor's plan to perform the Statement of Work; 2) Past performance information - relevant experience to perform the required services; and 3) fully loaded labor rate and anticipated hours. All responses will be evaluated on a best value basis using the following evaluation criteria: 1) Technical Understanding and Approach, 2) Personnel Qualifications, 3) Management, and 4) Price. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price and other factors). All responses/quotes submitted shall remain valid for a period of 180 days. For the purpose of establishing timely receipt of a quotation, the time will be established based upon the hard copy submission. Offerors are advised that CMS is a secured facility and therefore, offerors should allow time to obtain the appropriate security clearances (e.g., parking pass, visitor badge, etc...). Vendors are also advised that the CMS mailroom accepts only U.S. Postal Service mail. When utilizing an overnight carrier, you are cautioned that you must allow time for the proposal to be delivered to the exact mail stop noted above by the carrier by the time specified in this RFQ. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. All questions concerning this solicitation shall be submitted in writing no later than 10:00am (local Baltimore, MD time) on November 3, 2009. All relevant questions will be addressed in a timely manner, and the government's response will be provided to all interested parties with copies of both the questions and answers being supplied without reference as to the originating source. Questions submitted by telephone will not be entertained.
