- Attachment 2 - Project Statement of Work (SOW) Attachment 1 - Pricing Schedule (18 RFQ - 10-T-0044) Combined Synopsis and Solicitation - HC1019-10-T-0044 This is a COMBINED SYNOPSIS/SOLICITATION for commercial items and services prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both. The RFQ number is HC1019-10-T-0044. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-34 and DFARS Change Notice 20090115. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The NAICS code is 423610 and the Small Business Standard is 500 employees. This procurement is for Electronic and Information Technology (EIT). Effective June 25, 2001, the Federal government implemented Section 508 of the Rehabilitation Act of 1973, Amendments of 1998 (29 U.S.C. S 794(d)). Section 508 requires that the federal government only acquire electronic and information technology goods and services that provide for access by persons with disabilities. Therefore, to the extent supplies and services are available in the commercial marketplace, the contractor is required to meet the relevant accessibility standards contained in 36 C.F.R. Part 1194 (Subpart B). For further information see http://www.section508.gov. "One or more of the items under this acquisition is subject to Free Trade Agreements." The proposed contract is 100% set aside for small business concerns. Award will be made only if the offeror, the product/service or the manufacturer meets qualification requirement at time of award, in accordance with FAR clause 52.209-1 or 52.209-2. Quotes will be evaluated on the basis of low cost technically acceptable..Quotes shall include the following: Price Proposal: Vendors shall provide pricing for each CLIN Item (1 and 2) provide a Grand Total of all CLINS combined listed in Attachment 1. In addition, pricing for CLIN 1 will include a material list (Mfr P/N and Unit Cost) for each of the 18 electrical run. For CLIN 2, Labor shall include the estimated hours and labor rate, and the number of Installers to perform the job. The Contractor must be a Licensed Electrical Contractor registered and licensed in the State of Hawaii. A copy of the Contractor's License will be provided with their proposal. The Contract and all employees shall be U.S. Citizen. Responses shall clearly indicate the capability of the quoter to meet all specifiations and requirements, including schedule of installation and product availabilty. Copy of 52.212-3 and its ALT I as described above in applicable clauses. A determination by the Government to not compete this proposed effort on a full and open competitive basis, based upon responses to this notice is solely within the discretion of the Government. Oral communications are not acceptable in response to this notice. All responsible sources may submit a quote which shall be considered by the agency. Central Contractor Registration (CCR). Quoters must be registered in the CCR database to be considered for award. Registration is free and can be completed on-line at http://www.ccr.gov/. All quotes shall include price(s), FOB point, a point of contact, name and phone number, GSA contract number if applicable, business size, and payment terms.
