Description
The Government anticipates the award of one firm fixed price contract for NSN: 6540-01-578-7494, Mold Set, Carrier, Lens, Ophthalmic, in accordance with Medical Procurement Item Description #1 (MPID #1), and with all applicable requirements of MPID No. 4 dated 31 October 1996, covering the requirements for 6540-01-264-1348, and as specified in the solicitation. The Mold Set shall be designed to produce a minimum of 1,500,000 prescription lens carrier’s (PLC), NSN 6540-01-264-1348. The Mold Set shall consist of the following components: One eyewire injection mold with 6 cavities (3 left and 3 right eyewire), One injection mold with 4 cavities for hinge block, One screw hold taping tool, One spring retaining hole drive fixture, and all additional tools required to manufacture the PLC. The contractor must supply detailed instructions for producing the PLC. A Hot tip manifold design shall be used. The Eyewire groove shall be produced by the mold during the production process. The contractor shall provide one complete copy of all technical data to the Government. The technical data submitted shall include, but shall not be limited to, all specifications, drawings, and any other information associated with the production of the molds. All molds, tools and technical data shall become the property of the Government. First Article samples are required. The contractor shall furnish the following to the Contracting Officer for evaluation and approval prior to manufacture: 10 completely assembled first article samples of the PLC, manufactured from the mold set, and one set of lenses as specified in the solicitation. The contractor shall deliver 10 units of Lot/Item PLC’s within 30 calendar days from the date of this contract to the Government at Defense Supply Center Philadelphia; DSCP-FSCA (Charles Reimer); 700 Robbins Avenue; Philadelphia, PA, 19111.
