Description
This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice Background Schools offer the most systematic and efficient means available to improve the health of youth and enable young people to avoid health risks. Schools provide an avenue for reaching more than 55 million students each year. Planned and sequential school health helps young people at each grade develop the increasingly complex knowledge, attitudes, and skills needed to avoid health risks. A model of school health includes eight interactive components: health education, physical education and activity, health services, mental health and social services, nutrition services, healthy and safe school environment, faculty and staff health promotion, and family and community involvement. National efforts to implement school health have been hampered by a lack of information on policies and practices related to school health. To address this need, CDC conducted the first School Health Policies and Programs Study in 1994. The 1994 SHPPS assessed five components of school health at the state, district, and school levels. In addition, health education and physical education were assessed at the classroom level. At the state and district levels, the 1994 SHPPS focused on policies and practices in grades K-12. At the school and classroom levels, the 1994 SHPPS focused on policies and practices in middle and high schools. In 2000, CDC conducted the second SHPPS. That study assessed all eight components of school health at the state, district, and school levels. In addition, health education and physical education were assessed at the classroom level. SHPPS 2000 focused on policies and practices at the elementary, middle, and high school levels. Purpose and Objectives The purpose of this contract is to develop and conduct in the year 2012 the fourth School Health Policies and Programs Study (SHPPS 2012). SHPPS 2012 will characterize all eight components of school health in grades K-12 nationwide in elementary, middle, and high schools. In addition, the study will identify the extent to which school policies and practices address critical school health issues and six categories of behaviors related to the leading causes of mortality, morbidity, and social problems among youth and adults in the United States: 1) behaviors that contribute to unintentional injury and violence; 2) tobacco use; 3) alcohol and other drug use; 4) sexual behaviors that contribute to unintended pregnancy and sexually transmitted diseases, including HIV infection; 5) dietary behaviors; and 6) physical inactivity. Project requirements The Contractor shall design, conduct, and report on a national study of school health policies and practices in grades kindergarten through twelve. Designing the study includes revising the existing SHPPS 2006 questionnaires, reviewing and pilot testing the questionnaires, developing a sampling plan, and developing computer-assisted survey technology. Conducting the study includes selecting a sample, assisting in development of the OMB package, selecting and training data collectors, obtaining site approvals, and collecting data. Reporting on the study includes transferring data to the Government, preparing methodological and progress reports, and collaborating with the Government in the preparation of reports, manuscripts, and other summaries describing the data collected from the study. The Contractor, as an independent organization and not as an agent of the Government, shall furnish all personnel, facilities, equipment, supplies, and materials necessary to design, conduct, and report on a national study of school health policies and practices in grades kindergarten through twelve. At the state and district levels, the data shall be collected primarily via web-based surveys administered to state and district personnel with paper-and-pencil mail surveys used as follow up. At the school and classroom levels, data shall be collected via on-site structured computer-assisted interviews with school personnel. Other data collection may include, but is not limited to, school observations and review of written materials, including records abstraction. Anticipated period of performance Intent is to award a cost plus fixed fee contract for three (3) phases. The period of performance will be 32 months from the date of award. Capability statement /information sought Interested businesses that believe they possess the capabilities necessary to undertake this project should submit a hardcopy and an electronic copy of their capability statement, addressing the information regarding respondents': (a) staff expertise, including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. Companies must demonstrate their ability and experience in providing these services within the last 36 months. References shall include company name, point of contact, telephone number, fax number, and email address. Capability statements shall identify the business status (e.g. small business, 8(a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantage business, and women owned small business) of the organization. Responses, both electronic and hardcopy, must be received by the due date. Capability statements will not be accepted after the due date/time.
