Description: DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Organization: Department of Veterans Affairs, VISN 20, VA Puget Sound Medical Center 2.Description of Action: Describe the action being reviewed and approved. This is a request for other than full and open competition for the procurement of Maintenance Agreement from Hologic, Inc. See transaction: 663-10-1-163-0004 3.Description of Supplies or Services: 1)Telephone diagnostic and repair support Monday through Friday 7:30am through 9:00 pm EST 2)Labor and travel Monday through Friday, 8:00am to 5:00pm local time, exclusive of holidays 3)Next business day, on-site response to a down unit. 4)Calls received after normal business hours will be dispatched during the next business day, excluding holidays. 5)All applicable software updates supplied. 6)Two preventative maintenance inspections per contract year done during normal business hours (Monday-Friday 8am to 5pm) 7)All replacement parts free including glassware (detector and HVPS/tube) This supports R&D. It supports the mission of the VA by supporting a number of research programs that are ongoing here at VA Puget Sound Medical Center. The delivery date is 10/01/2009 and the period of performance is one year. 4.Statutory Authority: ( X) 1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements; ( ) (2) Unusual and Compelling Urgency; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability, or Expert Services; ( ) (4) International Agreement ( ) (5) Authorized or Required by Statute; ( ) (6) National Security; or ( ) (7) Public Interest 5.Validity of Use of Statutory Authority: Hologic, Inc., is the only authorized service agency for this equipment in the state of Washington (see quote for details). 6.Actions to Promote Competition: Contracting Officer will work with Research & Development to find new sources to meet our needs. See line #5. 7.Determination of Fair and Reasonable Price: The anticipated price to the Government is $ 9315.00.
