Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36 as corrected. This solicitation is unrestricted. The NAICS code is 621111, with a size standard of $10.0 million. All services will be performed at the VA Medical Center (VASNHCS) in Reno, NV. The performance period of this contract will be November 1, 2009 through October 31, 2010 with a possible six month extension per clause 52.217-8. Only electronic offers will be accepted. Offers must include a completed price schedule, curriculum vitae for each physician, and a maximum of three contract references (with correct contact information). Price must include a unit price per half day clinic. Curriculum vitae will be used for technical evaluation based on skill sets and experience. Contract references will be used for past performance and must be within the last three years (September 16, 2006). The technical and past performance evaluations, when combined are considered significantly more important than Price for tradeoff purposes. The Statement of Work and Price Schedule are as follows: 1. Scope of Work. The contactor shall provide physicians certified by the American Board of Internal Medicine subspecialty cardiology to VA Sierra Nevada Healthcare System (VASNHCS). Services include a full range of cardiology services for outpatient care, including consultation, diagnosis, therapy and treatment, and prognosis of patients with cardiology disorders. Services will be performed at VASNHCS. 2. Specific Contractor tasks include: 2.1. A minimum of six half-day outpatient clinics will be held per month. The number of patients scheduled may vary in number depending upon patient workload (usually 8-10 patients). 2.2. In the event the contractor physician is unable to conduct a clinic for any reason; the contractor must notify the COTR and Specialty Clinic 60 calendar days prior to the scheduled clinic. 2.3. Compliance with VA by-laws, rules and regulations, which include documenting evaluation, treatment recommendations, and further care management in the VA electronic medical record in accordance with VASNHCS Directive MOIC-003-08. 3. Normal Work Hours. Clinics will be scheduled during the normal work hours of VASNHCS, which are Monday through Friday, 8:00 a.m. to 4:30 p.m. 4. Qualifications. 4.1. The Contractor will be responsible for ensuring that each physician providing services under this contract is fully trained and completely competent to perform the required services covered by this contract. Contractor physicians shall be at a minimum: 4.1.1. Licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia, 4.1.2. Board Certified or board eligible in Cardiology, and 4.1.3. Hold a current, unrestricted Medical License, and 4.1.4. Must meet medical staff criteria for initial appointment and reappraisal in accordance with VHA Handbook 1100.19, entitled Credentialing and Privileging (see Appendix 2) to practice medicine at VASNHCS prior to beginning work.
