- Amendment #1 to RFP: VA-243-09-RP-0122Page 2: October 8, 2009 Continuation of Amendment #1 to RFP: VA-243-09-RP-0122 from Page 1: (1)From the Statement of Work Section 3. Qualifications, the following language is inserted: All Contract Physicians assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. All Contract IR physicians must be licensed by the State Board of Medical Examiners of the State, Territory, or Commonwealth in which they are currently licensed. All contract physicians must be board certified by the American Board of Radiology to work under this contract. Personnel assigned by the contractor to perform the services covered by this contract shall be proficient in written and spoken English (38 USC 7402). Contractor's assigned personnel must be computer literate. And the following language is removed: All Contract Physicians assigned by the Contractor to perform the services covered by this contract shall be licensed in the State of New Jersey.
