Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Part-time Respiratory Therapist to provide 624 hours; approximately 12 hours per week of services for the Rapid City PHS, IHS Hospital in Rapid City, South Dakota. This will be an option year purchase order with the base year period of performance from Date of Award through September 30, 2010 and each of the four (3) option years will run concurrent with the fiscal years (October 1 - September 30). The contractor shall perform according to the statement of work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-037, shall be in writing by email. Please provide the postal mailing address or email address to mail the RFQ package to. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be October 21, 2009. The closing date of the RFQ package is November 5, 2009. This is a small Business set aside.
