Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-10-Q-0001. This procurement is 100% small business set-aside under NAICS code 621320 with a standard business size of $7.0M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36, effective August 11, 2009. Contractor shall provide a firm, fixed-price rate for non-personal Optometry Clinic services. The requirement calls for 208 hours of general optometry services, and 416 hours for Diabetic eye exam services. The period of performance for this requirement is November 1 2009 to October 31 2010. Estimated start date is November 1 2009. The closing date for receipt of quotes is October 16 2009. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on October 16 2009. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Offers will also be accepted by fax at (405) 951-3771. Please DO NOT email RFQ's. Qualified vendors will review the following and submit applicable information. OPTOMETRY CLINIC SERVICES STATEMENT OF WORK Providing quality patient care is the goal of Haskell Indian Health Center. In an effort to achieve this goal, the Contractor shall provide optometry services for the patients of the Haskell Indian Health Center. The Contractor shall perform the following activities: 1. Screen, diagnose, and treat patients for vision changes and eye diseases. 2. Maintain an unrestricted optometry license. 3. Provide staff to assist the optometrist. 4. Submit a weekly, biweekly, or monthly invoice for payment. 5. Provide services 12 hours per week or as agreed upon with Haskell Indian Health Center. Indian Health Service, Haskell Indian Health Center shall perform the following activities: 1. Provide adequate workspace. 2. Provide access to medical charts. 3. Provide optometry equipment and supplies needed to perform eye exams. 4. Schedule appointments for patients according to their needs. 5. Supervision will be provided by the Clinical Director at the Haskell Indian Health Center in Lawrence, Kansas. It is the responsibility of the Contractor to submit an invoice for payment. Payment will be made within 30 days of the receipt of the invoice at the Oklahoma City Area Office, Office of Finance and Accounting, and in accordance with the Prompt Payment Act. CONTRACTOR QUALIFICATIONS & DOCUMENTS REQUIRED: Contractor shall: 1. Have a current license to practice Optometry. 2. Have current medical insurance in the amount $1,000,000 per occurrence and $3,000,000 aggregate. 3. Apply for and be credentialed at Haskell Indian Health Center in Lawrence, Kansas. 4. Have his/her own reliable transportation to the Haskell Indian Health Center. 5. Complete and comply with the terms specified in the "Addendum to Declaration for Federal Employment" and "Agreement to a Temporary Waiver of Character Investigation." XII. CLAUSES AND/OR PROVISIONS: 52.223-5 Certification Regarding a Drug-Free Workplace. CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in FedBizOpps, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.farsite.hill.af.mil. The provisions of 52.212-1, Instructions to Offerors-Commercial Items (Nov 2007), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) completed "Temporary Waiver of Character Investigation" and "Declaration for Federal Employment" form (2) Medical Insurance in the amount of $1,000,000 per occurrence and $3,00,000 aggregate (3) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award. This can be done at http://www.ccr.gov). The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications; (3) compliance with Indian Child Protection requirements; (4) all other required documents. If items 2, 3 and 4 are all within the bid, then item 1 (price) will be the determining factor for award. Quotes shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2007) or indicate certifications in ORCA at https://orca@bpn,gov. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Oct 2008), applies to this acquisition. EVALUATION CRITERIA The due date for this requirement is October 16 2009. THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THESE EVALUATION CRITERIA AS FOLLOWS: A) registration in the Central Contracting Registration (CCR) database (reference www.ccr.gov ) B) Propose and provide an all inclusive hourly rate for services. The estimated quantity is 208 hours for general optometry services, and 416 hours for diabetic eye exam services. The estimated start date of award is November 1 2009.
