Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-09-T-0258. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. The North American Industry Classification System Code (NAICS) for this procurement is 335311. This requirement is unrestricted to all contractors. The period of performance will be for a base year plus four option years. Blanchfield Army Community Hospital, Fort Campbell, KY, has a requirement for an MRI Uninterruptible Power System (UPS) ANNUAL SERVICE MAINTENANCE CONTRACT : CLIN 0001 - Government owned UPS Annual Service Maintenance Contract located at Blanchfield Army Community Hospital (BACH), Ft. Campbell, KY (Equipment List and description of services to be provided are listed in the attached work statement.) Period of performance: 01 October 2009 - 30 September 2010. Unit Price__________ Total____________ CLIN 1001 - First option of MRI UPS service maintenance contract. Period of Performance: 01 October 2010-30 September 2011. Unit Price__________ Total_________ CLIN 2001 - Second option MRI UPS service maintenance contract. Period of Performance: 01 October 2011-30 September 2012. ADDENDUM TO 52.212-2 Paragraph (a) is hereby replaced with the following: 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Factor 1 Technical Capabilities demonstrated expertise and knowledge required to manage this effort to include interface with Government staff after personnel are in-place to determine customer satisfaction, ensure personnel adhere to contract requirement and proactively deal with potential problems affecting worker performance and mission accomplishment. The contractors demonstrated ability to provide an adequate quality control plan. Factor 2 Price Assessment of price reasonableness based on comparison to market prices for requested services, considering the level of experience required. Award will be made to the lowest price, technically acceptable proposal. ***If the number of proposals received exceed 5, the contracting officer may establish a competitive range in order to ensure an efficient source selection. If this occurs, only the 5 highest rated proposals will be included in the competitive range. A rating of satisfactory or better for each factor and subfactor must be received in order to be considered for award. (b)Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offerors specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
