This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-249-09-RQ-0209, is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 334510. This procurement is full and open competition. Quotations are due on or before September 18,2009 by 2:00 PM Central Standard Time to be considered responsive. ITEM 1: Routine preventative and corrective maintenance that provides all labor, materials, equipment, supervision, and travel expenses required to provided monthly preventative maintenance and emergency maintenance during normal working hours (8am-5pm) on the Med Rad injectors and related accessories located in Radiology. These services are required to be in place not later than October 1, 2009. This is a multi-year 5 year contract, beginning October 1, 2009 and expiring September 30, 2014. Contractor shall perform preventative/emergency maintenance as need as require by Radiology maintenance schedules/technical manuals as suggested by the manufacturer. Equipment includes:Stellant D/Ds-DCB-SCTD #20592;Spectris Solaris/EP-DCB-SMRS #42342;Mark V,Mark V+ Provis, DCB-PRO, #103807; Stellant D, #29391 Delivery shall be FOB DESTINATION to James H. Quillen VA Medical Center, Sydney and Lamont Street, Bldg 205 Mt Home, TN 37684. Offers are due by 2:00 PM CST on September 19, 2009. The assigned Purchasing Agent is Theresa Upchurch.
