- DESCRIPTION: This is a sources sought synopsis to determine the availability of potential sources/vendors having the skills and capabilities necessary to perform/provide the services required. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. This is a sources sought synopsis only. Only questions sent by email to teresa.cochran@va.gov will be entertained at this time. Telephone responses and/or inquiries will not be accepted nor responded to. Provide only the requested information below. Responses should be sent to: Overton Brooks VA Medical Center, Attention: T. Cochran P&C Office (90C), 510 E. Stoner Avenue, Shreveport LA 71101-4243 and must be received no later than NOON CDT, September 17, 2009. This notice is to assist the VA in determining sources only. A solicitation is not currently available; therefore, DO NOT REQUEST A COPY OF A SOLICITATION THAT DOES NOT EXIST. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hubzone, 8(a), small, small disadvantage, woman owned small business, or large business) relative to NAICS 562211 Hazardous Waste Treatment and Disposal (size standard of $12.5 million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. Should a solicitation be issued it would be for services beginning in January 2010. SERVICES REQUIRED: The Overton Brooks VA Medical Center is seeking sources for a potential contract to provide infectious/bio-hazardous, pathological and chemotherapy waste, pick up and disposal. A qualified contractor must be capable of furnishing all labor, transportation, equipment, containers' appropriately sanitized, disinfected and labeled, supplies and materials required to provide pickup, transport and incineration of approximately 1150 pounds of infectious/bio-hazardous waste, pathological and chemotherapy waste, daily (Monday through Friday) from the Overton Brooks VA Medical Center, located at 510 East Stoner Avenue, Shreveport, LA 71101. Note pathological and chemotherapy waste must be incinerated. REQUESTED INFORMATION: MUST BE RECEIVED BY SEPTEMBER 17, 2009 1. GENERAL INFORMATION ABOUT YOUR COMPANY to include: (a) Contact Information (name of point of contact, company name, address, phone number, fax number and email address), (b) Business Size (large or small); (c) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business; (d) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act; (e) Indicate if your firm is a certified Hub zone firm; (f) Indicate if your firm is a woman-owned or operated business; (g) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB); (h) Include the DUNS number of your firm.; (i) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. 2. DOCUMENTATION TO SUPPORT TECHNICAL CAPABILITY to include: (a) Copies of the following permits: DOT Permit for Hauling Bio-Hazard Medical Waste, Incinerator Permit From the State Health Officer, and Personnel Permits From the State Health Officer; (b) evidence of long term pollution liability insurance; and (c) a brief [twenty pages or less] capability statement explaining your capability to perform the services needed. Include past experience in performing infectious/bio-hazardous, pathological and chemotherapy waste, pick up and disposal (incineration) services for the VA, other Government (Federal or State) agency, or for a private medical facility in the past five years.
