Description
This is a Combined synopsis/Solicitation for commercial Items prepared in accordance with the format in FAR, Subpart 12.6 as supplemented with additional information Included in this notice. This notice constitutes the only solicitation issued. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA-247-09-RQ-0337 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-36. NAICS is 325412. The Department of Veterans Affairs Birmingham, AL requires the following; STATEMENT OF WORK: (1) Contractor shall provide all Labor, Compounding, Supervision, and Transportation necessary to provide daily deliveries to Nuclear Medicine Service (HOT LAB), Room 2340, Department of Veterans Affairs Medical Center, 700 South 19th Street, Birmingham, AL 35233 as requested/ordered by Nuclear Medicine personnel/Physicians. Flat rate unit doses (30-35 millicuries) of Generic Cardiolite and/or Cardiolite/Sestamibi in syringes, precalibrated as requested pior to delivery. Deliveries shall be made between the hours of 7:00am and 4:30pm, Monday through Friday (excluding Federal Holidays) to include source and/or calibration dose, as requested with no additional charge. The Contractor shall provide daily deliveries as needed and/or requested by Nuclear Medicine personnel. No additional charges will be accepted for deliveries and there will be no limit on the number of deliveries made to the VA-Nuclear Medicine service. Bids will be accepted for flat rate doses of Generic Cardiolite and/or Cardiolite/sestamibi in syringe{s). Usage is estimated. (2) The vendor will be responsible for compliance with all federal and state regulatory agencies (DOT, OSHA, EPA, NRC) as applicable to policy, regulations regarding radioactive materials. (3) All radiopharmaceutical orders must be within + /- 10 percent (%) of the doctor's order allowing only for correcting calibration date and time.(4) All radiopharmaceutical orders must be in total compliance with the State of Alabama and Federal regulating agencies to include, NRC, FDA, DOT, OSHA, and EPA, USP 797 and 823, etc regarding radioactive materials. (5) All shipping containers must be free of radioactive contamination both inside and outside and shall meet and be in compliance with all state and federal regulating agencies (ie., Department of Transportation (DOT), and Nuclear Regulatory Commission (NRC) as required by law. (6) Contractor must supply with each shipment/order/delivery a system for "SPENT" (Used) syringes, needleless and meets OSHA's Biohazard Policy and regulations. (7) EMERGENCY DELIVERY: In case of an emergency, the Contractor will be required to make emergency deliveries within one (1) hour, excluding weekends and federal holidays as requested by OVA Nuclear Medicine Service personnel. (8) LICENSE: A copy of the DVA Medical Center, Nuclear Regulatory Commission (NRC) License will be furnished to the contractor upon award of the contract. (9) PLEASE NOTE: If contract vendor cannot supply dose due to technical or mechanical problems, Contract vendor is required to buy dose from other source and transport to the VA Medical Center to assure veteran(s) patient care is not compromised at the VA Medical Center. (10) In the event of this happening, the RSO - Kathy Boyd must be notified of this action, at extension 6610 or she may be reached by digital pager 205-917-0694. The RSO must assure regulatory compliance within the federal and state regulations, and any other agencies involved with the transport of radioactive isotope doses within our facility.(11) ADDITION/REDUCTION/CANCELLATION: The DVA shall have the option to add, reduce and/or cancel an order(s) by giving the contractor at least one (1) hour notice. (12) No additional charges will be accepted and there is no limit on the number of deliveries made to the VA Medical Center. (13) Contract Vendor must notify personnel (VA, Nuclear Medicine/Fiscal Service) of any problems that arise in regard to billing /unpaid invoices within 60 days of order date. If vendor fails to notify VA unpaid monies will be forfeited. It is the vendor's responsibility to bill appropriately and timely and notify if problems arise with billing issues.THE SCHEDULE OF PRICES FOR THE PERIOD OF 10/01/2009 THROUGH SEPTEMBER 30, 2010 SHALL BE LISTED AND PRICED AS FOLLOWS: Estimated Doses 4,000 ea. "Radiopharmaceutical Doses Generic Cardiolite and/or Sestamibi" $ ________ea. $_________ Total. Submit offers on Company letterhead stationary in accordance with FAR 52.212-1 Instructions to Offerors commercial items. Acceptance time will be 60 days from offer due date. The following provision apply to this solicitation: 52.212-1, 52.212-3 Representations and Certifications-Commercial Items; 52.212-4 Contract Terms and Conditions; 52.204-7 Central Contractor Registration; 52.212-1 Solicitation Provisions Incorporated by Reference; 852.270-4 Commercial Advertising; 852.271-70 Nondiscrimination in Services Provided Beneficiaries; 52.252-2 Clauses incorporated by reference 52.232-18 Availability of Funds; 52.232-38 Submission of Electronic Funds Transfer Information with offer; 52.233-2 service of Protest; 52.233-4 Applicable Law for Breach of Contract Claim. Interested parties are required to submit prices, clear and convincing data substantiating their capabilities and qualifications to satisfy this requirement. The Government contemplates award of a firm fixed price single award contract resulting from this solicitation. All offerors responding to this announcement shall submit an offer along with the information required in 52.212-1 and a completed copy of provision 52.212-3, to Contracting Officer, Central Alabama Veterans Health Care System, East Campus 2400 Hospital Road, Bldg. 68 Rm. 215, Tuskegee, AL 36083 by 2:00PM CST, 09/15/2009.
