- This is a Combined Synopsis/Solicitation for commercial Items prepared in accordanceWith the format in FAR, Subpart 12.6 as supplemented with additional information Included in this notice. This notice constitutes the only solicitation issued. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA-247-09-RQ-0329 and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-36. NAICS is 325412. The Department of Veterans affairs Birmingham, AL requires the following; STATEMENT OF WORK: (1) Contractor shall provide all labor, compounding, supervision, and transportation of Radiopharmaceutical Doses of F-18 Fluorodeoxyglucose (FDG) calibrated as requested in syringes, to the Department of Veterans Affairs Medical Center, (VAMC) Nuclear Medicine Service located at 700 South 19th Street Birmingham, AL 35233 as requested/ordered by Nuclear Medicine personnel/physicians. Doses will be delivered in syringe pre-calibrated [5-20 millicuries] to include dose for source/calibration use as requested and/or for linearity check included in price quoted. Delivery charges must be estimated into the per dose charge. No additional delivery charges will be paid. All Charges are for the dose as per the order of the Nuclear medicine personnel/physicians. (2) Delivery will be daily, pre-calibrated as ordered Monday through Friday 7:00am 4:30pm (excluding Federal holidays and weekends). (3) All radiopharmaceutical orders must be within +/- 10 percent 10% of the doctors order allowing only for correcting calibration date and time. (4) All radiopharmaceutical orders must be in total compliance with the State of Alabama and Federal regulating agencies to include NRC, FDA, DOT, OSHA, And EPA, USP 797 and 823, etc. regarding radioactive materials. (5) Contractor must supply with each shipment/order/delivery a system for spent syringes and is needleless that meets all regulating agencies as stated above and meets OSHAs Bio-Hazard Policy/Regulations. (6) No additional charges will be accepted. No fuel Surcharges and no limit on the number of deliveries made to the Nuclear Medicine service. Usage is estimated. (7) NOTE: If contract vendor cannot supply dose, due to technical/mechanical problems/failure, vendor must still provide the doses through another vendor without additional charges to the VA. The VA RSO must be contracted if another vendor is used to ensure regulatory compliance. Vendor must purchase dose and deliver to the VA, Nuclear Medicine Service, Contacting the Radiation Safety Officer at (205) 917-0694 to ensure regulatory compliance. (8) Contractor must notify designated personnel (VA, Nuclear Medicine/Fiscal Service) of any problems that arise regarding billing/unpaid invoices within 60 days of order date. If vendor fails to notify VA of problems, vendor will forfeit unpaid monies without further compensation. (9) Failure of Vendor to bill appropriately, monthly, in arrears and by government purchase order number may cause the contract to be cancelled. (10) Emergency order: In case of an emergency, the contractor will be required to make deliveries within one [1] hour, (excluding weekends and federal holidays), as requested by the Nuclear Medicine physicians. (11) License: A copy of the DVA Medical Centers Radioactive material permit will be furnished to the contractor upon award of the contract. (12) Additions/Reduction/Cancellation: The DVA Medical Center/Nuclear Medicine Service shall have the option to add, reduce or cancel an order(s) by giving the contractor at least one [1] hour notice. (13) The VA Medical Center has the option to return (FDG) F-18 Fluorodeoxyglucose doses in the original container with the patients name in the event of a patient not able to keep appointment due to deceased, too ill to travel, etc., and we can receive a credit for up to five [5] doses per month. The SCHEDULE OF PRICES FOR THE PERIOD OF 10/01/2009 THROUGH SEPTEMBER 30, 2010 SHALL BE LISTED AND PRICED AS FOLLOWS: Estimated Doses 850 ea. "Calibrated FDG Doses" $ ________ea. $_________ Total. Submit offers on Company letterhead stationary in accordance with FAR 52.212-1 Instructions to Offerors commercial items. Acceptance time will be 60 days from offer due date. The following provision apply to this solicitation: 52.212-1, 52.212-3 Representations and Certifications-Commercial Items; 52.212-4 Contract Terms and Conditions; 52.204-7 Central Contractor Registration; 52.212-1 Solicitation Provisions Incorporated by Reference; 852.270-4 Commercial Advertising; 852.271-70 Nondiscrimination in Services Provided Beneficiaries; 52.252-2 Clauses incorporated by reference 52.232-18 Availability of Funds; 52.232-38 Submission of Electronic Funds Transfer Information with offer; 52.233-2 service of Protest; 52.233-4 Applicable Law for Breach of Contract Claim. Interested parties are required to submit prices, clear and convincing data substantiating their capabilities and qualifications to satisfy this requirement. The Government contemplates award of a firm fixed price single award contract resulting from this solicitation. All offerors responding to this announcement shall submit an offer along with the information required in 52.212-1 and a completed copy of provision 52.212-3, to Contracting Officer, Central Alabama Veterans Health Care System, East Campus 2400 Hospital Road, Bldg. 68 Rm. 215, Tuskegee, AL 36083 by 2:00PM CST, 09/15/2009.
