THIS IS A SOURCES SOUGHT, HOWEVER IF YOUR COMPANY CAN PROVIDE THIS SERVICE, PLEASE PROVIDE A QUOTE FROM THE INFORMATION ON THIS RFI. THIS SERVICE IS FOR THE VA MEDICAL CENTER, FAYETTEVILLE, ARKANSAS 72701 Contract will begin October 1, 2009 through September 30, 2010 STATEMENT OF WORK Hoya ConBio Dental laser & Versa Wave Laser Dental uses two different and unique lasers for specialized dental procedures. Both require special tools and contain many high cost, complex components. To assure these lasers are operational and provides safe, effective service, they require: 1.Emergency repairs, including technical telephone support and unlimited on-site emergency repair visits. 2.Coverage for all parts, travel, labor and shipping costs. 3.Parts including typical user replaceable filters. Tips, hand pieces, sleeves and accessories are excluded. 4.Telephone response time should be less than one hour. 5.Shipping to and from the manufacturers repair site needs to include overnight shipping to reduce down-time.
