Description

All work performed and material provided shall per the Habitability Materials Listed Rev K, MIL-STD-1623, FEDSPEC DDD-C-95, and military specifications as applicable. The Contractor shall provide a certificate of conformance in the format of the Habitability Certificate. HABITABILITY CERTIFICATE The Contractor is required to provide a "Habitability Certificate" with the supplies delivered under the contract. This certificate shall originate and be signed by the manufacturer of the material. In no case shall the Government's right to inspect supplies under the inspection provision of this contract be prejudiced. The signed certificate shall be attached to or included on the top copy of the DD250, DD1155, or SF1449 copies distributed to the payment office. When acceptance is at destination, a copy of the signed certificate shall also be attached to or entered on copies of the DD250, DD1155, SF1449 or other appropriate documents accompanying the shipment. This certificate shall read as follows: I hereby certify that on _________(date), (Contractor's name)provided the supplies called for by contract number ________________via ____________________(carrier) on ________________(bill of lading orshipping document) in accordance with all applicable requirements for shipment. I further certify that the supplies are of the quality specified and are in all respects in conformance with the contract requirements, including MIL-STD-1623 (SHIPS) and other specifications, preservations, packaging, packing and marking requirements; physical item identification (part number), and in the quantity shown on this or the attached acceptance document.
