THIS IS A COMBINED SYNOPSIS/SOLICATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only publication. A written solicitation will not be issued. Solicitation number M6700109Q0353 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-31. This solicitation is set aside 100% for small business. The North American Industrial Classification System (NAICS) code for this requirement is 711130 and the small business size standard is 500 employees. FRANCIS XAVIER CATHOLIC CHAPEL, CAMP LEJEUNE, NORTH CAROLINA FOR THE PERIOD 01 OCT 2009 THROUGH 30 SEPTEMBER 2010. TERM OF SERVICE: 01 OCT 2009 30 SEPT 2010 Organist/Pianist Scheduled Services: 52 Organist/Pianist Scheduled Rehearsals: 52 Special/Memorial Services/Rehearsals (NTE): 20 Total Services: Location: St. Francis Xavier Catholic Chapel, Bldg 17, Camp Lejeune, North Carolina PERFORMANCE BASED WORK STATEMENT FOR ROMAN CATHOLIC MUSICIAN IN SUPPORT OF SATURDAY RELIGIOUS SERVICES AT THE ST. FRANCIS XAVIER CATHOLIC CHAPEL, CAMP LEJEUNE, NORTH CAROLINA BACKGROUND: Provide Roman Catholic Musician services in support of the St. Francis Xavier Catholic Chapel Saturday religious services and the Marine Corps Base Command Religious Program for worship and other events as determined by the Marine Corps Base Chaplain Department. 1.0TASK: The contractor shall perform Roman Catholic Organist and/or Pianist music services and is required to support the missions and responsibilities of the Marine Corps Base Camp Lejeune Chapel Command Religious Program. The services shall include, but are not limited to the following: 1.1. Perform 52 Saturday Roman Catholic Organist and/or Pianist music services from 01 October 2009 to 30 September 2010 to active duty personnel and all other authorized as defined in OPNAVINST 1730.1D (Religious Ministries in the Navy). The Saturday Roman Catholic Mass and Holy Days of Obligation Organist and/or Pianist services shall be conducted each Saturday and are included in the total units requested. Dates and times are TBD. The services shall last for approximately (1) hours. 1.2. Provide organ and/or piano music in support of the regularly scheduled services at the appointed chapel. Arrive 15 minutes before each scheduled event. 1.3. Provide support of up to 20 additional services which may be scheduled during the church calendar year or civic holidays (as directed by the Catholic Chapel Priest). Plan, coordinate and provide music for worship, funerals, memorials, and special events. Services for Weddings are not authorized. 1.4. Provide 52, once weekly rehearsals for approximately one hour. Rehearse and lead all volunteer musicians including adult choirs, childrens choirs, other instrumental ensembles and vocal or instrumental soloists. Provide required support and/or musical rehearsals as scheduled by the Catholic Chapel Priest in support of regularly scheduled and additional worship services including: Easter, Christmas, ceremonies, and other services to complete the mission of the Command Religious Program. One scheduled service rehearsal will be conducted on one week night per week. The time and location of the rehearsal will be at the discretion of the Chapel Priest. 1.5. Notify the Catholic Chapel Priest and Officiating Chaplain by telephone or via email when unable to perform services due to scheduled vacation, illness, inclement weather and/or emergencies. Notification for any of the above will be in advance and 1.6. (below) will be performed in commensurate. Notification of vacations will be submitted 2 weeks in advance. 1.6. Provide and compensate a pre-approved substitute when unable to perform services required as depicted in this Performance Based Work Statement. 1.7. All invoices are submitted through WAWF and verified time cards must be submitted no later than the 5th of each calendar month to the Marine Corps Base Chaplains Department 1.8. Apprise the Catholic Chapel Priest of any malfunctions of the instruments and any required services needed to maintain the instruments musical integrity. Funding requests for necessary supplies and literature are submitted through the Chapel Priest. 1.9. A thirty-day notice is required prior to termination of this order by either party. 1.10. In performance of the above functions, the Organist/Pianist must coordinate services with the Catholic Chapel Priest and Officiating Chaplain prior to each service. 2.0 PLACE OF PERFORMANCE.
