Description
This is a combined synopsis/solicitation for commercial servicesprepared in accordance with the format in FAR 12.6, with additionalinformation included in this notice. This announcement constitutes theonly solicitation; proposals are being requested and a writtensolicitation will not be issued. Solicitation documents andincorporated provisions and clauses are those in effect through FederalAcquisition Circular 2005-36 as corrected. This solicitation is setaside for Small Businesses. The NAICS code is 813920, with a sizestandard of $7.0 million. The Contractor will provide these services atthe VA Central California Health Care System in Fresno, CA. Thecontract performance period will be for one year starting on or aboutOctober 1, 2009 through September 30, 2010, with a one-year option. Electronic, hand-delivered or properly mailed offers will be accepted.Faxed offers will not be accepted. Offers are due toeric.toliver@va.gov by NLT 3:00 p.m. on September 16, 2009. Offers mustinclude a completed price schedule, curriculum vitae for eachpharmacist, and a maximum of three contract references (with correctcontact information). Price must include an hourly rate for on-callcall backs. Curriculum vitae will be used for technical evaluationbased on skill sets and experience. Contract references will be usedfor past performance and must be within the last three years (September16, 2006). For information regarding the solicitation you can e-mail meat the above e-mail address or phone at (916) 923-4528. The Statement of Work and Price Schedule are as follows: 1. Scope of Work 1.1. The Contractor shall provide Pharmacist services to the VA CentralCalifornia Health Care System (VACCHCS) located at 2615 E. Clinton Ave,Fresno, CA for the period October 1, 2009 through September 30, 2010.Pharmacist services shall be provided on an as-needed basis inaccordance with the terms and conditions of this contract at the hourlyrates specified in the Price Schedule. The Contractor's price includesall necessary travel time, mileage charges and per diem to and from theContractor's place of business. 1.2. Pharmacy services include the interpretation of physician orders,dispensing of appropriate medications, and counseling patients onmedication administration and storage. The overall duties require acomprehensive knowledge in current clinical practice in patient drugtherapy emphasizing geriatric and rehabilitation medications anddelivery systems. The Contractor will, when assigned, rotate througheach operational area and/or shift of the Pharmacy Service. TheContractor is expected to comply with all mandated standards andrequirements. 2. Specific Contractor Tasks: 2.1.Clinical: 2.1.1. Provides accurate information for all inquiries from patients,family members, health professional, students, etc. Will provide followthrough and resolution to an acceptable end. 2.1.2. Effectively counsels a patient on drug interactions, adverse sideeffects, food-drug interactions, and compliance issues. 2.1.3. Provides consultations on proper drug regimens,contraindications, drug interactions, product availability and pharmacypolicies/procedures, Monitors and assesses various prescribed therapies(e.g., aminoglycoside antibiotics, antipsychotics, digoxin,theophylline, etc.) using pharmacokinetic techniques and will assistClinical Specialists in monitoring their assigned areas. 2.2.Distributive: 2.2.1.Evaluates patients' medication orders and dispenses appropriatemedications and dosage forms, properly labeled after reviewing forcompleteness, eligibility and appropriateness of drug, dose, route,strength, and quantity to be dispensed. Reviews for drugincompatibilities, inappropriate combinations, potential adverse drugreactions, dosage ranges, intra-agency polypharmacy practices andinitiates cancellation of non-active or duplicate prescriptions. Inputsappropriate information into the VA computer system (Outpatient, UnitDose, IV Admixture). 2.3.Administrative: 2.3.1. Ensures the Chief of Staff, the Chief Pharmacy Service, or theirdesignee is informed of drug inventory which includes monitoring ofeffective stock levels, expiration dates, proper storage conditions, andinventory control procedures. 2.3.2. Prioritize work, adjust dispensing/preparation activities, andaccomplishes effective clinical and distributive services. 2.3.3. When assigned, performs other duties that are deemed essential tothe pharmacy operation. 2.4. Normal Duty Hours: VACCHCS normal duty hours are 7:30 am to 4:30 pmMonday through Friday. The Contractor will provide service on an asrequired basis as scheduled by the VA Chief, Pharmacy Services.Typically Contractor hours will be Monday through Friday from 9:30 am to6:00 pm or 8:00 am to 4:30 pm. However there may be occasion when theContractor will required to work outside these hours. The contractorwill not be required, except in case of emergency, to furnish suchservices on a national holiday. VA observes the following 10 holidays: New Year's DayLabor DayMartin Luther King's BirthdayColumbus Day Washington's BirthdayVeterans DayMemorial DayThanksgivingIndependence Day Christmas 2.5. A Contractor's employee must be present at the VA facility andmust be actually performing the required services for the periodspecified in the contract. 2.6. A record keeping system of Contractor hours shall be established.The payments for any leave, including sick leave or vacation time, arethe responsibility of the Contractor. 2.7. The contractor's personnel shall agree to comply with the PrivacyAct of 1974 (the Act) and the VACCHCS rules and regulations issued underthe Act during and after execution of this contract. Any informationabout an individual that is maintained by the VA, including, but notlimited to education, financial transactions, medical history, andcriminal or employment history or any record that contains the person'sname, or the identifying number, symbol, or other identifying particularassigned to the individual, shall be kept CONFIDENTIAL. 2.8. The Contractor Pharmacist shall work and communicate effectivelywhile remaining courteous, tactful, and understanding when interactingwith others, even in potentially difficult or volatile situations. 3. Qualifications: 3.1. Contractor shall provide the services of a fully trained registeredand licensed Pharmacist. 3.2. Perform the services covered by this contact, the Contractor'spersonnel shall be licensed in a State, Territory, or Commonwealth ofthe United States or the District of Columbia. Qualifications shall besubject to the review by the VA Chief of Staff and approval by the VAfacility Director. 3.3. The Contractor's personnel shall be responsible and able to showevidence of reliability, ability and experience. Verified references ofability and qualifications may be accepted by the Contracting Officer tosatisfy these requirements. 4. Payments The Contracting Officer's Technical representative (COTR), inconjunction with the Contractor, shall keep accurate records of hoursworked. Based on subject records, the Contractor shall prepare properdocuments for payment for services rendered. Payment shall be mademonthly in arrears upon receipt of a proper invoice from the Contractor. 5. VA provided materials/services: The VA will provide the Contractor's employee with all pharmaceuticalsupplies and facilities, including VA operatinginstructions/regulations, without limitation; uniforms where applicableand necessary to enable the pharmacist to perform each and everyconfirmed work assignment in a clean, safe, and professionallysatisfactory manner. Under no circumstances will the pharmacist suppliedunder this contract be responsible for failure of performance due in anyway to lack of supplies or facilities. 6. Changes after award The Contracting Officer is the only person authorized to approve changesor modify any of the requirements under this contract. The Contractorshall communicate with the Contracting Officer on all matters pertainingto contractor administration. Only the Contracting Officer is authorizedto make commitments or issue changes that will affect price, quantity,or quality of performance of this Agreement. ln the event the Contractoreffects any such change at the direction of any person other than theContracting Officer, the change shall be considered to be made withoutauthority and no adjustment will be made in the contract price, terms,or conditions to cover any increase in costs incurred as a result thereof. 7. STATUS OF INDEPENDENT CONTRACTOR The relationship between the VACCHCS and the Contractor's personnel isnot and shall not be construed or interpreted to be a partnership, jointventure or agency. The relationship between the VACCHCS and theContractor's personnel is an independent contractor relationship.
