Description
SOW PHYSICAL EXAMINATION Pricing Sheet Fill in Wage Determination RFQ Fill In This is a combined synopsis/request for proposals for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotations are requested and a written solicitation will not be issued. The government anticipates award of one base year with four option years if exercised by the government. Evaluation of options shall not obligate the Government to exercise the option(s). This solicitation will incorporate provisions and clauses that are in effect through the latest Federal Acquisition Circular 2005-35 effective July 14, 2009. The North American Industry Classification System code is 622110-General Medical and Surgical Hospitals, with a Size Standard restriction of 31.5 million dollars. The Corporation of National and Community Service will award a firm fixed price (FFP) purchase order contract for Physical Examinations, and Chest X-rays for 160 AmericCorps*NCCC North Central Region members. The Place of Performance is 1004 G Street, Vinton, IA 52349. "SEE ATTACHMENTS FOR SOW DETAIL" Award will be made to the offeror determined to be the Best Value to the Government in accordance with FAR 52.212-2 Evaluation Factors. Evaluation may be made with or without discussions with the offeror. Evaluation factors listed in descending order of importance: (1) Quality of services and number of practitioners available for exams; (2) Location (Distance from the campus) (3) Past Performance. (4) Quality of services and number of practitioners available for exams, location and past performance, when combined, are more important than price. All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting price and technical proposals and descriptive literature. Technical proposal should consist of literature describing services offered. Past Performance should include information for physical examinations and comprehensive medical services provided for groups similar in both size and scope and at least three references who can verify quality of your services (provide name and telephone numbers). All responses to these requirements must be submitted on letterhead. Any offer that does not meet the solicitation requirements will be rejected as non-responsive. Prices will be evaluated separately from the technical proposal and will be evaluated in terms of quality of services, number of practitioners available for exams and location. The provisions at 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition. The vendor shall include a complete copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with your quote. Clause 52.212-4 Alternate I, Contract Terms and Conditions-Commercial Items and Clause 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order-Commercial Items are incorporated by reference. Addendum to 52.212-1 Instructions - quotations are due no later than 3:00pm/1500p.m. (EST) on Tuesday, September 8, 2009. Quotations shall be submitted electronically to Mr. Leroy Dawson at: ldawson@cns.gov. It is the Offeror's responsibility to ensure the offer is received (in total) by the Contract Specialist by the deadline. Questions shall be submitted to Mr. Leroy Dawson at: ldawson@cns.gov.
