Description
This is a synopsis for a commercial acquisition using simplified acquisition procedures (Reference FAR Parts 12 and 13). THIS IS A SYNOPSIS AS REQUIRED BY FAR PART 5.2 - THIS IS NOT A REQUEST FOR QUOTE (RFQ). VENDORS SHOULD NOT SUBMIT FORMAL QUOTES/PROPOSALS UNTIL THE RFQ IS POSTED TO THIS WEBSITE. 45 CONS/LGCB has a requirement for the following professional medical staff to commence work on 01 October 2009 for a period of twelve (12) months: - Full time Radiologist (shall have graduated from an approved school and completed a residency or fellowship acceptable to the US Air Force Surgeon General, shall possess a valid unrestricted license to practice diagnostic imaging as a radiologist from the State of Florida, shall be board eligible or board certified by ACR and have provided a minimum of five (5) years of service and a minimum of six (6) months within the past twelve (12) months), one each - Part time Radiologist (shall have graduated from an approved school and completed a residency or fellowship acceptable to the US Air Force Surgeon General, shall possess a valid unrestricted license to practice diagnostic imaging as a radiologist from the State of Florida, shall be board eligible or board certified by ACR and shall have provided five (5) years services and a minimum of six (6) months within the past twelve (12) months), one each - Part time Social Worker (shall possess a Masters of Social Work degree or be a Clinical Psychologist and must have graduated from an approved school and completed a residency or fellowship acceptable to the US Air Force Surgeon General, shall possess a valid unrestricted license to practice social work or psychology from the State of Florida, shall be board eligible or board certified and shall have provided psychotherapy services a minimum of twelve (12) months in the last three (3) years as a therapist in the outpatient mental health or primary care setting), one each - Full time Registered Nurse for the Family Practice Clinic (shall have and maintain an active, valid, current, unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse (RN) in a U.S. jurisdiction, all licenses must be unencumbered and remain in effect during contract employment, shall be a graduate of a nationally accredited associates degree, diploma or baccalaureate degree program in nursing, shall hold current certification as a registered nurse (RN), shall have at least twenty four (24) months of experience within the last seven (7) years. This experience shall have been in a primary care clinic setting. Nurses that work in the areas of critical care (ICU, CCU, NICU), operating room, labor & delivery or emergency room shall have twenty-four (24) months of experience within the last sixty (60) months), two each - Full time Physical Therapy Assistant (shall have graduated from a physical therapy assistant program accredited by the American Physical Therapy Association (APTA) and have at least twelve (12) months of experience within the last three (3) years), one each This service will be conducted at Patrick AFB, Florida. The base period of performance will be 01 October 2009 - 30 September 2010. Four one-year options may be awarded. The government intends to make a single award to the lowest priced, technically acceptable offeror. The geographical restriction of this competition will be limited to 8(a) certified firms. All other firms are deemed ineligible to submit offers. Solicitation No. FA2521-09-T-0143 is issued as a Request for Quotation. It is advertised with a Standard Industrial Code (SIC) of 8011, business size of $10.0M and a North American Industry Classification System (NAICS) code of 621111. All responsible sources may submit a proposal, which shall be considered by the agency. The solicitation will be issued electronically on or about 08 September 2009 at www.fbo.gov.
