Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36 as corrected. This solicitation is unrestricted. The NAICS code is 621111, with a size standard of $10.0 million. The delivery, acceptance, and FOB point is the VASNHCS in Reno, NV. The contract performance period will be for one year starting on or about October 1, 2009 through September 30, 2010. Only electronic offers will be accepted. Offers are due to joel.burgess@va.gov by NLT 3:00 p.m. on September 14, 2009. Offers must include a completed price schedule, curriculum vitae for each physician, and three contract references (with correct contact information). Price must include a weekly price for inpatient/on-call services and a clinic price for half-day outpatient/procedure clinics. Curriculum vitae will be used for technical evaluation based on skill sets and experience. Contract references will be used for past performance and must be within the last three years (September 11, 2006). Scope of Work.The contactor shall provide board certified or board eligible physicians in Critical Care and/or Pulmonary Medicine to provide pulmonary/critical care to VA beneficiaries at VA Sierra Nevada Health Care System (VASNHCS) in Reno. Services consist of the full range of consultation services for pulmonary/critical care for inpatients and outpatients including consultation, diagnosis, therapy and treatment, and prognosis of patients. Services also include the supervision and education of resident physicians and medical students from the University of Nevada, School of Medicine (UNSoM). 2. Specific Contractor tasks include: 2.1. Inpatient Care.Inpatient services will be performed approximately six months out of the year at the VA in 14 day increments and includes night and weekend on-call coverage. See Paragraph 2.10 below for on-call services. Service includes: 2.1.1. Supervise residents and direct resident workup and treatment of patients. 2.1.2. Conduct rounds seven (7) days a week (typically three to four hours per day). Rounds include seeing all non-cardiac inpatients identified by the Intensive Care Unit (ICU) attending physician in the ICU and Telemetry Units daily. Document the evaluation of the patient's progress and any directions for further evaluation and management of the patient's care in the patient's electronic medical record. See Paragraph 2.7 below regarding Medical Records. 2.1.3. Inpatient Pulmonary/Critical Care Consultation: Provide attending level evaluation of inpatients needing pulmonary/critical care evaluation. 2.1.4. Provide teaching rounds one hour a day, three times a week to ICU internal medicine residents. 2.2. Outpatient/Procedure ClinicsOutpatient services will be performed four to eight hours per week year round. Outpatient service includes: 2.2.1. The Contractor shall provide one or two four-hour Outpatient or Pulmonary Procedure Clinics at VASNHCS each week contingent upon patient workload. Pulmonary Procedure Clinics will include bronchoscopy(s) for either inpatients or outpatients. Patients for either type of clinic will be scheduled by VASNHCS. 2.2.2. The Contractor shall perform the equivalent of two bronchosocpy procedures or see a maximum of ten patients per clinic. 2.2.3. The day and time of each clinic will be set a minimum of 30 days prior to the scheduled clinic to allow time for scheduling patients. The dates and times for clinics will be approved by the Chief, Medical Service or designee after reviewing the Contractor's recommended schedule submission. The Contractor's schedule shall include the date, time, and assigned provider for each clinic. A full explanation will be provided to the Contractor for any date or time that has been disapproved. An alternate date and time will be mutually agreed upon. 2.2.4. In the event that a clinic is cancelled for any reason, the COTR shall be notified immediately and the clinic will be rescheduled after a new date and time has been approved. In the event the contractor cannot reschedule a clinic the VA reserves the right to reduce the amount equivalent to the reduced service. 2.3. The Contractor physician shall be responsible for providing patients with information about risks, benefits alternative treatments and obtaining the informed consent of the patient/family needed before conducting procedure, as well as, management of conscious sedation. 2.4. The Contractor physician shall provide professional direction to the support staff regarding all procedures he or she may participate in. 2.5. Standards. Comply with all applicable VA guidelines; medical center medical staff bylaws, rules and regulations; hospital polices/procedures; and in accordance with current Joint Commission standards. 2.6. Medical Records. The contractor physician shall document evaluation and treatment recommendations in the VA electronic medical record in accordance with VASNHCS Directive MOIC-003-08. All progress notes, consults, discharge summaries, and/or clinical and surgical procedure reports, etc. must be completed (including signature) within 24 hours. 2.7. Quality. Contractor physicians shall participate in facility Quality Improvement and Safety Programs, as well as peer reviews when required by the Chief, Medical Service or designated representative. Additionally the contractor physician shall participate in continuous performance improvement activities as required. 2.7.1. To enhance patient care and satisfaction performance measures will be established by VASNHCS or implemented as mandated by the Veterans Health Administration (VHA) and/or the Sierra Pacific Network (VISN 21) as goals. The Contractor shall adjust their work effort to meet these goals. 2.7.2. Performance measures include VA interest items such as timely appointments for new and follow-up patients, quantity of patients seen per day, clinical practice guidelines, preventive medicine, and Undersecretary monitors. Performance measures and monitors may change as patient needs arise. The contractor will be provided with the table of measures and monitors annually or as changes occur. 2.8. Normal Work Hours. VASNHCS Normal work hours are Monday through Friday, 8:00 a.m. - 4:30 p.m. Work hours may vary from week to week in order to meet the needs of VASNHCS patients, 2.9. On-Call Coverage. Contractor physicians will be responsible for providing on-call coverage to VASNHCS for consultation and/or emergency services 24 hours per day while performing inpatient services (approximately six months per year) in compliance with VASNHCS Clinical Practice Guidelines, Joint Commission standards, and VHA guidelines. 2.9.1. Consultation. Contractor physician consultation to on-site residents and/or VA staff can be accomplished by phone after normal work hours and weekends and holidays, unless the clinical situation warrants patient contact. 2.9.2. Response Times. Contractor physicians shall be available 24 hours a day by pager for emergencies, and be physically present in the Medical Center to see a patient within 45 minutes. 2.10. Supervise and instruct residents and medical students per the curriculum of the UNSoM and in accordance with VHA Handbook 1400.1, entitled Resident Supervision, dated July 27, 2005. This includes documentation and confirmation of all resident and student patient evaluations. 2.11. All services will be accomplished within the VA Outpatient Clinic, Medical-Surgical wards, the Intensive Care Unit, and the Telemetry Care Unit at VASNHCS. 3. Qualifications. 3.1. The Contractor will be responsible for ensuring that each physician providing services under this contract is fully trained and completely competent to perform the required services covered by this contract. Contractor physicians shall be at a minimum: 3.1.1. Licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia, 3.1.2. Board Certified or eligible in the practice of Critical Care and/or Pulmonary Medicine, 3.1.3. Hold a current, unrestricted Medical License, and 3.1.4. Must meet medical staff criteria for initial appointment and reappraisal in accordance with VHA Handbook 1100.19, entitled Credentialing and Privileging (see Attachment 2) to practice medicine at VASNHCS prior to beginning work. This handbook also describes the process for reappraisal and reprivileging, as well as reduction and revocation of privileges.
