Description
STATEMENT OF WORK 1. DESCRIPTION OF SERVICES The Samuel S. Stratton VA Medical Center in Albany, New York has arequirement for the proper containment, removal and disposal,disinfecting and re-installation of sharps containers at 113 HollandAvenue, Albany, New York as well as its VA Primary Care OutpatientClinics located in Bainbridge, New York, Fonda, New York, Catskill, NewYork and Clifton Park, New York. 1)Containers are to have direct drop with a top feed.2)Containers must be installed, collected, emptied, disinfectedand re-installed by the contractor.3)Contractor is required to adjust container locations and pickupsto meet the unique and changing requirements of each clinical area inthe medical center as well as the outpatient clinics.4)All containers must be emptied before becoming seventy fivepercent (75%) full or within thirty (30) calendar days of the last emptydate, whichever is first.5)Reusable sharps containers must be properly disinfected and mustbe sample tested to show that a 6 log reduction of vegetative organismand a 3 log reduction of mycobacterium are met.6)Contractor must provide a log of all pickups, including physicaldescription of containers at the time.7)Contractor must provide Material Safety Data sheets (MSDS) onany chemicals it uses in the medical center or outpatient clinics.8)Performance must comply with all state, federal, local, JCAHO,EPA and all other regulatory agencies.9)Contractor may NOT subcontract more than 49% of work to beperformed.10)Contractor must supply the VA with all training records of itsstaff as well as mandatory training requirements.11)Contractor is required to subject its employee's to any and alltraining required and conducted by the VA (i.e. orientation training,emergency preparedness and any other training the VA may require ofoutside contractors)12)Contractor's employees must provide proof of currentvaccinations for influenza, Hepatitis B and C, Measles, Mumps, andRubella.13)Contractor's employees must undergo a security backgroundinvestigation in accordance with VA policy, and provide proof of drugtesting.14)Contractor removes 28 gallon needle receptacle contained insteel container for public sharps disposal program.
