Description
The providence VA Medical Center is seeking interested and capable source to determine a set-aside for the anticipated contract for pharmacy claim processing. The Providence VA Medical Center needs access to pharmacies that function 24 hours a day, 7 days a week, and 365 days a year to provide medication to veterans when the Providence VAMC Outpatient Pharmacy is closed or for the Community Based Outpatient Clinics (CBOC's). The pharmacies chains being proposed must have a location within 10 miles of each location listed below; Providence VAMC, 830 Chalkstone Ave, Providence RI, 02908; Hyannis CBOC, 233 Stevens St, Hyannis, MA 02601; Martha's Vineyard CBOC, Martha's Vineyard Hospital, Hospital Rd, Martha's Vineyard, MA 02557; Middletown CBOC, One Corporate Place, Middletown, RI 02842; Nantucket CBOC, Nantucket Cottage Hospital, 57 Prospect St., Nantucket, MA 02554; New Bedford CBOC, 175 Elm St, New Bedford, MA 02740. The pharmacy will provide up to 5 day supplies for non-controlled substances and controlled substances, and up to 10 day supplies for antibiotics. The exception would be inhalers, insulin, topical products that should be dispensed in the lowest available package size. Drugs shall abide with the current VA National Formulary. Any capable, interested vendor should provide the following information, the names, address and hours of the pharmacies to be used within 10 miles of each location. The VA is planning on soliciting a contract with a base 10/1/2009 - 09/30/2010 and two option years. The NAISC code for this is 524291 and the size standard is $7.0 million.
