PLUS 20 HOURS OF ON SITE LABORTERMS AND CONDITIONS 1. Items 1 thru 5: Two (2) complete preventive maintenance inspections will be made on each room and mobile units including, but not limited to, complete generator calibration per manufacturer's specs; inspection and/or repair of all x-ray tube suspension system, including all cleaning, lubricating and proper operation of safety features. All cleaning, lubricating and necessary adjustments will be made on tables. Collimators, image intensifiers, etc, will be checked and adjusted as needed. Complete documentation will be provided on inspection items. The preventive maintenance inspections (PMI's) will be performed during the month of February 2010 and August 2010 unless authorization is otherwise granted through Engineering Service. NOTE: A detailed PM list is included. 2. Item 5: Service is to include up to 20 hours of Service Representative time on the group of equipment listed. Full telephone engineering support on minor problems will be available. All parts required will be supplied through local service engineers. Excludes: all glass. Item 3 & 4: These items will be on PM service only. Item 1 & 2: These items are full service maintenance. Excludes: camera coverage, x-ray digital coverage, image intensifier coverage, and other options. 3. Emergency calls will be responded to within 24 hours. Call in service will be provided Monday thru Friday, during normal working hours, excluding holidays. 4. For payment purposes the contract year will be divided into twelve (12) job periods (one job period for each month of the calendar year) beginning October 1, 2009 and ending September 30, 2010. Payment in the amount of 1/12 of the contract will be made at the end of each job period, which includes any required emergency repair visits and the preventive maintenance inspections. The PM visit will be considered to be worth 100% of the job period. Payment will only be made after a service ticket is presented for each service visit and for the required PM visit. INVOICES WILL BE HONORED FOR EACH JOB PERIOD IN ARREARS. 5. INSTRUCTIONS FOR SERVICE REPRESENTATIVES: Performance of services is to occur between 8:00 a.m. and 4:00 p.m., Monday thru Friday, excluding Federal Holidays unless special permission is granted through Engineering Service at Danville. EMERGENCY CALLS ARE TO BE RESPONDED TO WITHIN 24 HOURS OF THE INITIATING CALL, unless other permission is granted. The service representative is to report to the Program Specialist's office, Engineering Service, Bldg. 64 to sign in before making the service call. At the end of each preventive maintenance visit, the service representative must initial its completion on the service report. At the end of other than preventive maintenance service, the service representative must present a service ticket indicating the service performed and parts used. All service tickets must be delivered to the Program Specialist's office, Engineering Service, Bldg. 64 when the service representative returns at the end of the visit to sign out. Copies of all service tickets must be on file in Engineering Service before invoices will be certified for payment. Any part and/or service not covered by this service contract WILL NOT BE INSTALLED WITHOUT FIRST PROCURING A FORMAL PURCHASE ORDER FROM THE VA MEDICAL CENTER, ENGINEERING SERVICE, DANVILLE, IL. 6. CONTRACTOR'S RESPONSIBILITIES: The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/She shall take all precautions necessary to protect persons and property from damage during performance of this contract. He/She shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal property that occurs during the performance of this contract that is caused by him/her or his/her employee's fault or negligence. The contractor shall maintain personal liability and property damage insurance prescribed by the laws of the State of Illinois. X-RAY GENERATOR 1.Clean XRC,H.V. transformer & P.U. 2.Check operator indicator lights, P.B.'s and knobs. 3.Inspect hand-switch(s), foot-switch(s), and cable(s). 4.Clean and regrease HV cables for rad tube. 5.Clean and inspect rad tube cooling fan and cables. 6.Calibrate generator to manufacturer's specifications. 7.Make adjustments and repairs as necessary to insure safe and proper operation. X-RAY TUBE SUSPENSION 1.Clean tracks, rails, inspect and/or repair cable drape(s), concealments. 2.Clean and lubricate bearings as necessary. 3.Clean and lubricate wear strips. 4.Check all locks-adjust if needed (electrical and mechanical). 5.Inspect all steel cables for wear and proper tension. 6.Inspect/repair all electrical cables. 7.Insure proper operation of all functions (safety systems, indicators, scales). X-RAY TABLE 1.Clean table inside and outside. 2.Clean and lubricate all pulleys, bearings, bearing surfaces, and rails. 3.Inspect tabletop drives, locks, and steel cables. Make adjustments as necessary. 4.Check bucky and bucky tray for proper operation. 5.Inspect/repair all electrical cables. 6.Check table angulation operation and chain (belt) drive.FLUORO TABLE 7.Inspect operation of spotfilm device and fluoro carriage. 8.Check/adjust locks and all moving parts. 9.Inspect/calibrate fluoro tube, collimator, and spotfilm device to H.H.S. and manufacturer requirements.10.Clean tube fan. COLLIMATOR 1.Check/adjust light to x-ray fields. 2.Check/adjust field size indicators. 3.Tighten mounting screws. 4.Check cone tracks and extension cylinder. IMAGE INTENSIFIER 1.Inspect and adjust camera to manufacturer specifications. 2.Adjust 1.1. for best image. 3.Clean all lenses. 4.Adjust automatic brightness for proper operation. 5.Adjust monitor for best image. 6.Check all electrical cables. 7.Check monitor support. (cart-suspension) Make minor repairs as necessary during the inspection and provide a list of other problems that need attention.
