- Bid Schedule Past Performance Questionnaire Performance Plan Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F2U4009176A001. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36 and DAC DCN 20090729. This procurement is being issued as 100% small business set-aside. The North American Industry Classification System (NAICS) Code is 621399 and the size standard of 7.0 million applies to this procurement. The requirement is for 24-Hour Nurse Triage Service for Luke AFB, AZ. The period of performance is 12 months with four twelve month option periods. This requirement is subject to the availability of funds. Funds are not currently available for this requirement. The following clauses and provisions are applicable to this solicitation and can be viewed through internet access at the FAR Site at http://farsite.hill.af.mil. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The evaluation will be based on past performance and price. Past performance is significantly more important than price. Please submit three past performance references with your quote using the attached past performance questionnaire. Past performance must be recent (within the past three years) and relevant to nurse triage services. Offerors shall include with their quotes, a completed copy of FAR 52.212-3, Offer Representation and Certifications - Commercial Items at website http://orca.bpn.gov. If representations and certification have been completed, please provide DUNS number. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
