Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, Purchase #F2Z4M89205A003, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-36. This solicitation is set aside for small buisness's. The NAICS code is 541611 and the small business size standard is $7,000,000.00. The following commercial items are requested in this solicitation; 0001-Computer Radiography Maintenance - Services, non-personal, service agreement for 2 each computed radiography (CR) readers serial #36726480B and #36726481B to include 2 each IIP, serial #30BKZ21 and #70KBZ21. Serviceagreement to include scheduled and emergency service, one preventive maintenance inspection, all parts, labor and travel of technicians, all software updates, facotry trained Fugifilm field service engineers, one hour telephonic response and 4 hour on-site response for hard down systems within 150 miles. 0002- Bone Densitometer Maintenance - Services Non-Personal, service agreement for bone densitometer, Model: Excell, Serial No: 1637 0003- X-Ray Maintenance - Service nonpersonal, maintenance service agreement for Phillips X-ray system. To include unlimited second response labor and travel coverage M-F (8am-5pm) excluding holidays, priority response service, unlimited parts coverage, customer service documentation license, customer service diagnostic software license, unlimited remote technical assistance, operating system software updates. 0004- Digital Mammography System Maintenance - Services Nonpersonal, service agreement for digital mammography system (ImageChecker DX and CheckMate) to include priority scheduling, unlimited emergency service, unlimited parts, preventive maintenance, factory authorized product updates and modifications, factory trained and authorized service engineers, extensive parts inventory, technical and applications support. 0005- Maintenance for Microscan - provide complete equipment maintenance (travel, labor and parts) five days per week, M-F, between the hours of 7am-5pm local time for the AS4/IBM Microscan Autoscan-4 system at 57950 Leavenworth St, Ste 3L1, Lab Clinic. After hours and holidays do not apply. Also included are regularly scheduled preventive maintenance (PM) visits of one per year. 0006- Lab Analyzer service maintenance agreement. For ADVIA Lab Analyzer Model 120, Serial # IRL40710736. 0007- Ultrasound Maintenance - Services, Non-personal, service agreement for ultrasound machine to include piority scheduling, unlimited emergency service, unlimited parts, preventive maintenance, factory authorized product updates and modifications, factory trained and authorized service engineers, extensive parts inventory, technical and applications support. The period of performance for the items 0001-0007 above is 28 Sep 09 - 27 Sep 10. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. AFFARS 5352.201-9101, Ombudsman (10 Aug 2005) in full text (below): (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMBC A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen, Mr. Michael R. (d) The ombudsman has no authority to render a decision that binds the agency.
