- Recreation Therapy Services Statement Of Work 1.To provide offsite Recreation Therapy Assessment Services to include but not limited to: Assessment Services would include a Certified Therapeutic Recreation Specialist (CTRS) that would be available to evaluate identified veterans who have sustained injuries related to military service and those veterans who incur significant physical disability post discharge from service resulting in substantial barriers to previous leisure activities. Services to be rendered in the veteran's home and/or community setting. The compliment of CTRS assessment services required is as follows: Present physical/functional limitations and abilities. Premorbid medical and functional status. Previous leisure skills assessment. Social and Mental Health issues as they pertain to barriers. Pain assessment. Cognitive assessment. Adaptive equipment needs assessment. 2.To provide Therapeutic Recreation interventions to include but not limited to: Therapeutic interventions would include a Certified Therapeutic Recreation Specialist (CTRS) who would travel to the veteran's home and/or community setting where the leisure activities will be performed. Transportation of veterans as needed. The compliment of services required is as follows: Wheelchair transportation for clients. Access to a wide variety of adaptive recreation equipment to utilize in a variety leisure activities and sports. Actual skill practice with adaptive equipment, as indicated, to optimize performance in the specific activity identified. Counseling and education in community resources and information. List of CPT Codes that are part of this contract. 97110-Assestment.97535-Self Care/Home Management Training 97537-Community/Work Reintegration 97542-W/C Management/Propulsion Training97113-Aquatic Therapy97530-Therapeutic Activity97532-Development of Cognitive Skills.The initial assessment will be rendered on a case by case visit.. all other visits will be offsite. 3.Procurement Of Adapted Recreation EquipmentAll requests for adapted recreation equipment will be submitted through the COTR, or designee, for review. The COTR, or designee, will enter a prosthetics consult and the equipment will be delivered to the contract group. These providers will be responsible for proper training and documentation of such in CPRS before issuance to the veteran. Adapted equipment will only be requested after trial of this equipment as part of the veterans training program.4.Scheduling AppointmentsAll appointments will be communicated to SPRS program Assistant to enter into VISTA scheduling package prior to the visit. All no-shows will be documented in CPRS and reported to the SPRS Support Assistant same day the appointment was missed. All cancellations made prior to the appoint time will be reported to the SPRS Support Assistant so as to remove the visit from the scheduling package prior to the appointment date/time.
