- SOL: VA-261-09-RP-0206: Due September 4, 2009 Request for Proposal - Ambulance Services for VA Palo Alto Health Care System, San Francisco VA Medical Center and VA Northern California Health Care Systems, to include associated outpatient clinics. This contract is expected to have a one year base period with four option years thereafter. This is a presolicitation announcement, the actual solicitation with descriptions/specifications/work statement should be posted on or about August 19, 2009. This acquisition is a full and open competition. All responsible bidders may submit an offer which will be considered by the agency. NAICS Code is 621910, size standard $7.0 million. Product Service Code is V225. Telephone requests will NOT be accepted. All questions must be submitted in writing, questions may be faxed to W. Ulibarri, (650) 849-0332 or emailed to bill.ulibarri@va.gov. You may also mail to William Ulibarri, Consolidated Contracting Activity (CCA/90), VAPAHCS, Room C-140, Bldg. 6, 3801 Miranda Ave., Palo Alto, CA 94304. Electronic solicitation may be accessed at no cost at http://www.fedbizopps.gov. Any amendment issued against this solicitation will also be available from the same URL address. Contractor is responsible for submitting all documents required in accordance with the solicitation, including any amendments which will be posted on this website. Solicitation is also subject to availability of funding. Proposals shall be submitted on an all or nothing basis. Partial proposals will be rejected. Proposal shall be valid for 60 days from offer due date. The Contracting Officer may include any additional contract requirements(s) or terms and conditions determined by the Contracting Officer to be necessary for this acquisition and consistent with customary commercial practices.
