Small Business Set-Aside: Total Small Business
Description: CLINs Spreadsheet Statement of Work The Uniformed Services University of the Health Sciences (USUHS), the DoD Medical School and Health Sciences University in Bethesda, MD intends to procure, on a competitive basis, certified service/maintenance on three Gaylord hood ventilator systems. This notice serves as a combined synopsis/solicitation for commercial items prepared in accordance with the procedures in Subpart 12.6, as supplemented with additional information included in this notice. This solicitation will be issued as a request for quote (RFQ). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-36. The North American Industry Classification System (NAICS) code is 561790 (Other Services to Building and Dwellings) and a small business size standard (in millions of dollars) of $7.0. It will be the responsibility of the contractor to check FedBizOpps (http://www.fbo.gov) for any amendments to the solicitation. The government will award a firm-fixed-price (FFP) contract resulting from this solicitation to a responsible offeror. The requirements for this service/maintenance are included in the attached statement of work (SOW). Please note, the contractor must be certified to work on Gaylord Hood Ventilator systems. Award will be made based price. The contractor shall provide pricing for the line items within the attached CLINs excel spreadsheet. Please complete the spreadsheet with your quoted price and submit spreadsheet as part of your quote. There will be only one comprehensive site visit on Tuesday, August 25, 2009 at 2:00 PM EST at the Uniformed Services University of the Health Sciences, Bethesda, MD in Building A Room 1040C. Attendees should allow adequate time to obtain access onto the USUHS campus location. Due to heighten security measures, National Naval Medical Center (NNMC) is requiring everyone entering the base to have a government issued ID card, military ID, or ID card issued by a Federal Government activity/agency. Those who are not employed by the federal government, who therefore do not have official government identification; will be required to be placed on NNMC's access list. In order to be placed on the access list please provide the name of the attendees, company's name, and phone number. Also, please have proper identification such as a valid driver's license or passport. All requests must be received by Friday, August 21, 2009 at 5:30 PM EST to be assured access onto the base. USUHS intends to award a purchase order for the service/maintenance for a base period of twelve (12) months; in addition four (4) one year options to continue to provide service/maintenance. (d) Comptroller General Examination of Record.
