- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0394 is issued as a Request For Quotation (RFQ). Doc# W81NTE9160N183 This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005.26. This acquisition unrestricted under NAICS code 811219, Size Standard $7M. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: CLIN0001: NON-PERSONAL SERVICE MAINTENANCE FOR EP MED MONITORING SYSTEM; MDL: EP WORKMATE 9301-15-P-01 AND TO INCLUDE ALL COMPONENTS, SOFTWARE UPGRADES AND CERTIFICATIONS. SERVICE VENDOR SHALL PROVIDE ALL PARTS, MATERIALS, LABOR AND EQUIPMENT NECESSARY TO PERFORM MAINTENANCE IN ACCORDANCE WITH MFG SERVICE/REPAIR MANUALS FOR CERTIFING, CUSTOM PROGRAMMING AND INCLUDES CHANGES, MODIFICATIONS, OR ENHANCEMENTS TO THE SYSTEM Service hours 24/7 day AND MAINTAIN MFG WARRANTY. Delivery date is Period of Performance will begin 01 OCT 2009 thru 30 Sep 2010. Place of delivery and acceptance is for Brooke Army Medical Center, Ft Sam Houston Tx. FOB destination is preferred. FAR provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall include a completed copy of FAR provision 52.212-3 as outlined therein. EVALUATION OF QUOTES: a. The Government will award a contract resulting from this solicitation to the responsible quoter with satisfactory past performance whose quote, conforming to the solicitation, is Lowest Priced, Technically Acceptable (LPTA). The following factors will be used to evaluate quotes: 1. Technical acceptability: Explain how your company will meet the characteristics of the contract line item numbers (CLINs) as specified in the solicitation. 2. Price. 3. Quoters must submit their quotes via fax.4. Evaluation Process: All quotes will be evaluated on overall technical acceptability. The award decision will be based on the lowest priced technically acceptable quote. The Government will evaluate quotes for award purposes by adding the total price for basic requirement. The Government may determine that a quote is unacceptable if the prices are significantly unbalanced. Evaluation shall not obligate the Government. FAR clause 52.212-4, Contract Terms and Conditions- Commercial Items, applies to this acquisition to include the following addenda: Defense Federal Acquisition Regulation Supplement (DFAS) 252.232-7003-Electronic Submission of Payment Request and DFAS 252.211-7003.
