- COURIER SERVICE STATEMENT OF WORK Contractor to provide all labor, equipment, transportation, gasolineinsurance, and anything else needed to provide courier service for theDepartment of Veterans Affairs, Manchester, NH and to the destinationsmentioned below.The contractor shall be expected to pick up and deliver items such aslab specimens, laboratory transport containers, patient records, medicalsupplies, housekeeping supplies and any other items required. Forbilling purposes-it is requested that should driver be picking up labsupplies and medical records at same destination and same day andtime-the pickup be done by same driver in order to avoid duplicatebilling. OBSERVENCE OF HOLIDAYS. This contract does not include holidays. The following is list ofFederal holidays that will be observed. New Years Day, Martin LutherKing Day, Presidents Day, Memorial Day,Independence Day, Labor Day,Veterans Day, Thanksgiving Day and Christmas Day. INSURANCE REQUIREMENTS Contractor shall meet all Federal State and City codes regardingoperation of this type of business and furnish the required InsuranceCertificates required to work on a Government installation. Theserequirements will be described in the Special Contract Requirements section. INCLEMENT WEATHER-Contractor shall contact the Lab Supervisor first andthe Contracting Officer second should he feel the weather conditionswill delay or cause cancellation of services. Due to nature of thiscontractor it is necessary that contractor furnish vehicles that areable to drive in difficult weather circumstances. This station must benotified within 2 hours of pick up time should pickup be impossible.Cancellations should only as a last resort due to acts of God. TOLLS-Costs of all tolls must be included in the bid price DRIVERS-All drivers must be of legal age and maintain a neat, clean andprofessional appearance. Driver must be able to speak and understand andwrite English They must be licensed in accordance with Federal State andLocal transportation authorities. Drivers should wear clothing bearingcontractors company name or wear a badge showing contractor name and jobtitle. The contractor is responsible for the actions and conduct of itsemployees and shall assure that they are compliant with all VeteransAdministration building regulations, policies and guidelines while onthe premise. Driver shall not make any personal stops in performance ofduties under this contract nor shall any unauthorized passengeraccompany the driver during contract work. Drivers shall not park inareas that haven't been authorized by the station. Drivers shall observethe smoking policy of the Government and only smoke in designated areas.Drivers transporting lab specimens must be trained in UniversalPrecautions in handling of biohazard materials and include usingclinical procedures in case of spills or accidents. This training shouldalso include mandatory Hazardous materials training. All Hazardoustraining and a statement from the transportation service that meet lawsand regulations of the Dept. Of Transportation and the Centers ofDisease Control governing the transport of clinical specimens besubmitted yearly to the laboratory manager. All the training completedshould be submitted to the Laboratory Manager annually. VEHICLES-All vehicles used in performance of this contract shall belicensed and meet the minimum vehicle requirements as mandated by theDept. Of Transportation (DOT). The Government reserves the right toinspect the vehicles and investigate the responsibility of bidder toperform this contract and reject any offer should contractor be lacking. CONFIDENTIALITY OF RECORDS- Contractor shall assure that theconfidentiality of all patient records being transported and shall beheld liable in the event of any breach. The contractor shall comply withprovisions of the Federal Privacy Act of 1974 (Public Law 93-579).REPORTABLE INCIDENTS-Contractor shall furnish a written report shouldany delivery incidents occur. This report should be delivered to thisoffice within 24 hours of the incident. Incidents/actions involvinglaboratory specimens must be reported immediately to Lab Manager.Incidents/actions involving breach of patients records or patientidentifiable information must be reported immediately to either theInformation Security Officer or Privacy Officer.EVIDENCE OF RESPONSIBILITY-Contractor must have minimum of five yearsexperience and possess all the necessary equipment and manpowernecessary to perform this contract. Contractor be financiallyresponsible and possess all the licenses necessary to perform thiscontract.QUALITY CONTROL AND ASSURANCE- Contractor shall develop and maintain aQC program to assure the above qualifications are met. Contractor mustshow that are able to remedy any violations of the above are rectified.The Government reserves right to perform periodic inspections to assurecompliance.CONTRACTING OFFICER ROLE- The Contracting Officer or designee is theonly person authorized to make any changes to this agreement. Thisincludes changes to price, quantity and performance. Should any otherperson make changes, they will be regarded as void and no adjustment tocosts will be considered.PERSONNEL POLICY-All employees working for the contractor shall not beconsidered as Government Employees, and are considered only employees ofthe contractor. The contractor shall provide Workers Compensation,Professional Liability Insurance, health Examinations, Income TaxWithholding and Social Security Payments. PACKING AND TRANSPORTATION-all specimens shall be properly packaged andlabeled to indicate the general nature of material being transported.They shall be packed in a container secured to vehicle body to preventmovement. Lab specimens are to be transported in accordance withtemperature requirements. Room temperature specimens will betransported inside a cooler and remain at temperature between 22-25 C.Refrigerated specimens must be transported in cooler packaged withregular ice and remain a temperature of 2-10C. Frozen specimens will betransported in cooler packed with dry ice and remain at temperature of-15 to -20C STAT PICKUPS-These calls or any calls requiring immediate pickup for anyLab specimens should be made within 15 minutes of the call with a totalturn around time of 30 minutes to the Eliot Hospital. Routine pickups ordeliveries shall be performed within 1 hour. Upon arrival the VA willhave a list of items being transported.
