Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81K00-09-T-0398 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This requirement is SB-set aside and all responsible sources may submit a quotation, under NAICS code 811219 and the business size standard is 500. The requirements in this solicitation are: CLIN 0001, NON-PERSONNEL SERVICE & MAINTENANCE FOR ANESTHESIA UNITS (5 EA) - AESTIV/5 ANES MACHINES; Anesthesia monitors 5 ea - AESTIVA 3000; Vaporizers 13 ea TEC 6. INCLUDES PREVENTIVE MAINTENANCE EVERY SIX MONTHS AS WELL AS UNLIMITED ON SITE REPAIR AS NEEDED. PERIOD OF PERFORMANCE: 01 OCT 2009 THRU 30 SEP 2010 MFG AESTIVA SITE LOC: FT RILEY KS.
