- The Request for Quotation H94002-09-Q-0018 is the written solicitation which contains the contract line items, and the specification and evaluation criteria with response worksheet. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; proposals are being requested and a written solicitation will be issued as request for quotation number H94002-09-Q-0018. The solicitation/request for quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36, effective on August 11, 2009. The NAICS Code is 511210 - Software Publishers with a business size standard of $25,000,000; this requirement is 100% small business set-aside. The requirement is for document management data migration tool software licenses with maintenance and support for one 12-month base period and four 12-month option periods. See the attached solicitation/request for quotation H94002-09-Q-0018 for the contract line items, and the specification and evaluation criteria with response worksheet. The delivery with performance period, date of award not to exceed 12 months, and acceptance will be at the FOB destination: TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, CO 80011-9066. The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: technical capability, and price. Technical and past performance, when combined, are approximately equal to price. Offerors are advised to include a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items, Alternate I, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, and the additional FAR clauses cited in the clause are applicable to the acquisition. The contracting officer has determined additional FAR and DFARS clauses and contract requirement to be necessary for this acquisition and consistent with customary commercial practices. Signed and dated proposals shall be received by August 28, 2009, no later than 3:00 PM Mountain Time. Proposals received after the date and time stated may be rejected.
