Description
This is not a solicitation announcement. This is a sources ought synopsis only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources and their size classifications relative to NAICS Code 621399. Responses to this sources sought synopsis will be used by the government to make appropriate acquisition decisions see requirements below. Travis AFB (David Grant Medical Center (DGMC) is seeking sources for a potential contractor to provide all necessary labor, tools, equipment, materials, parts and transportation required to provide Anesthesiology Services. Listed below are the qualification and requirements for ANESTHESIOLOGIST 1. QUALIFICATIONS 1.1 Current board certification by the American Board of Anesthesiology or the American Osteopathic Board of Anesthesiology. 1.2 CPR, BCLS, and ACLS certification must be obtained through the American Heart Association and/or Red Cross approved training program. 2. SPECIFIC TASKS The duties include but are not limited to the following: 2.1 The anesthesiologist shall perform essentially the same duties as those required of any Medical Corps Officer or government Civil Service physician of similar experience and in similar duty assignments. Services include, but are not limited to: 2.2 The anesthesiologist shall be qualified to provide routine and emergent anesthesia care for neonatal through geriatric patients with American Society of Anesthesiologists (ASA) classifications I - VI. 2.3 Specifically, the anesthesiologist shall provide regional and/or general anesthesia care for the following subspecialties: general surgery, obstetrics, gynecology, cardiothoracic, vascular, adult dentistry, pediatric dentistry, otolaryngology, neurosurgery, urology, ophthalmology, oral surgery, orthopedics, plastic surgery, podiatry, and dermatology. 2.4 The anesthesiologist shall also provide resuscitative care during a cardiopulmonary arrest, and sedation or general anesthetic care at remote locations. 2.5 The anesthesiologist shall personally provide this care, or he/she shall supervise the simultaneous delivery of anesthesia care by up to four certified registered nurse anesthetists (CRNAs); administer anesthesia in lieu of CRNA to patients whose condition poses a particular anesthetic difficulty. 2.6 Administer appropriate diagnostic and therapeutic nerve blocks in the treatment of acute and chronic pain. 2.7 Oversee the activities of the recovery room and administer appropriate drug and respiratory therapy in the recovery room setting. Implement the proper use of mechanical ventilators in the recovery room, and as necessary, in the intensive care units. 2.8 Provide consultation regarding preoperative preparation, intra and postoperative anesthetic management, and treatment of chronic pain patients. 2.9 The anesthesiologist shall consult with other members of the Local MTF medical and nursing staff to provide quality perioperative care. 2.10 The anesthesiologist shall document his or her medical care in accordance with Local MTF regulations, Air Force Instructions (AFI), and the standards established by the American Society of Anesthesiologists. 2.11 The anesthesiologist shall continue to provide ongoing anesthesia care until he or she is relieved by a qualified anesthesiologist or certified registered nurse anesthetist. 2.12 The anesthesiologist shall attend monthly Anesthesiology Department meetings (or Surgical Services Department meetings in the absence of an Anesthesiology Department), and quarterly MTF Medical/Dental Professional Staff meetings; if requested, the anesthesiologist will present medical lectures, lead discussions concerning medical topics, and teach students. Contractor's employees shall have the education, training and licensure, certification and experience necessary to provide services. The intended contract shall provide the Government the option to extend the contract in one-year increments for a base and four option years. Responses to this notice should include company name, address, point of contact, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) and must respond to the following questions: (1) Is your business a large or small business? (2) If small, does your firm qualify as a small, emergency business, or a small, disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and is your firm certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified "hub zone" firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned and Veteran-Owned? (7) Do your firm have an Environment Management System Certification (EMS)? Required Submittal: Contractors must submit the following: 1. A brief introduction into the company, summary of related capabilities information based upon the contract experience with Government or commercial customers listed below. 2. A summary of relevant contract experience, include a reference to the contract number, magnitude, place of performance, and points of contact for each example provided (name of the point of contact, address, e-mail, telephone number, fax number, and company's web page, if applicable). Submitted information shall be UNCLASSIFIED. 3. Any other information you think we need to evaluate your capabilities. Experience Documentation: The vendor shall provide a summary of experience, within the last 3 years, on performance and current customer operations for provided perfusion services. As a minimum, the summary shall include the following information for the projects proposed as relevant experience: 1) Customer Company Name, 2) Installation Location, 3) Date of Customer Acceptance, 4) Is the service still in use?, 5) Name of Customer Contact, 6) Telephone and Fax Number of Customer Contact, and 7) E-mail of Customer Contact. Any information submitted by respondents to this synopsis is strictly voluntary. The Government will not reimburse the respondents for any costs associated with their response. The Government reserves the right to contact additional customers to obtain information that will be used to evaluate the vendor's experience. This is not a request for proposals and in no way obligate the Government to award any contract. Responses to this notice shall be mailed to Bryan Armstrong, 350 Hangar Ave, Bldg 549, Travis AFB CA. Telephone and/or e-mail responses will not be accepted. Responses must be received in writing no later than 4:30 p.m. Pacific time,31 Aug 2009. Firms responding to this announcement which fail to provide all of the required information cannot be used to help the Government make the acquisition decisions which is the intent of this synopsis. THIS SOURCES SOUGHT IS FOR INFORMATION AND PLANNING PURPOSES ONLY AND IS NOT TO BE CONSTRUED AS A COMMITMENT BY THE GOVERNMENT TO A SET-ASIDE. NO REIMBURSEMENT WILL BE MADE FOR ANY COSTS ASSOCIATED WITH PROVIDING INFORMATION IN RESPONSE TO THIS ANNOUNCEMENT AND ANY FOLLOW-UP INFORMATION REQUESTS. RESPONDENT WILL NOT BE NOTIFIED OF THE RESULTS OF THE EVALUATION. ALL INFORMATION SUBMITTED IN RESPONSE TO THIS ANNOUNCEMENT MUST BE RECEIVED ON OR BEFORE THE CLOSING DATE
