- This is not a solicitation announcement. This is a sources ought synopsis only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources and their size classifications relative to NAICS Code 621399. Responses to this sources sought synopsis will be used by the government to make appropriate acquisition decisions see requirements below. Travis AFB (David Grant Medical Center (DGMC) is seeking sources for a potential contractor to provide all necessary labor, tools, equipment, materials, parts and transportation required to provide Anesthesiology Services. Listed below are the qualification and requirements for Clinical Perfusionist 1. Qualifications 1.1 Bachelor's degree with emphasis on medical technology, respiratory therapy, biological sciences, or nursing. 1.2 Graduated from an accredited training program in cardiovascular perfusion 1.3 Certified or eligible for certification as a Certified Clinical Perfusionist (CCP) by the American Board of Cardiovascular Perfusion (ABCP). 1.4 Three (3) years of clinical experience is preferred. 2 Specific Tasks 2.1 Assess, plan and implement the patient care requirements and treatment necessary for cardiopulmonary bypass/assistance before, during or after surgery. Operate the heart/lung machine to support the patient. 2.2 For Cardiopulmonary bypass: 2.2.1 Appropriately assess patient profile and manage cardiopulmonary bypass 2.2.2 Use appropriate equipment/supplies and monitor data based on patient profile (age, condition, procedure). 2.2.3 Appropriately communicate with clinical team during procedures 2.2.4 Recognize and resolve variances and abnormal situations that may arise 2.2.5 Utilize all available safety devices during cardiopulmonary bypass as per departmental policy 2.2.6 Demonstrate appropriate clinical vigilance and awareness before, during and after cardiopulmonary bypass 2.2.7 Facilitate discussion with health care team which includes medical record review, decisions needed for prime selection, drug and blood additives, techniques of cardiopulmonary bypass. 2.3 For Ancillary Procedures: 2.3.1 Demonstrate competence and comprehensive knowledge of all ancillary procedures 2.3.2 Competence in cardiac assist devices to include VADs, ECMO, IABP, PCPS, left heart bypass,VV bypass for liver transplant, hyperthermic isolated limb perfusion, and autotransfusion, to include platelet gel processing. 2.3.3 Assess patient profile and manage ancillary procedures by using appropriate equipment and monitoring data 2.3.4 Recognize when situations are critical, notify appropriate personnel and assist as needed 2.3.5 Troubleshoot all technical devices 2.3.6 Create and maintain accurate, complete and legible records. 2.3.7 Timely documentation according to departmental standards 2.4 Communicate product failures, technical and patient incidents 2.5 Complete and file necessary paperwork 2.6 Provides concise and accurate shift report 2.7 Maintain patient confidentiality Addendum Add to Specific Tasks: Shall perform all clinical perfusion duties associated with cardiac surgery, including operation of the cardiopulmonary bypass machines and cell saver for open heart surgeries. Prepare a detailed patient assessment after reviewing the case history, pertinent lab values and proposed surgical plan. Assembles and primes the CPB circuit, initiates, monitors, and terminates cardiopulmonary bypass, operates and monitors ultra filtration equipment. Accurately obtains and records vital signs, ACT and ABG during bypass procedures. Provide auto transfusion services for cardiothoracic and other surgical procedures. Assembles, operates and disassembles auto transfusion apparatus, processes auto transfusion bloods for perioperative blood salvage, and documents all cell salvage completely and accurately. Provide Intra-Aortic Balloon Pump (IABP) support as required. Assembles, operating assists with the insertion and removal of the IABP apparatus and provides troubleshooting support, timing and maintenance as needed. Provides perfusion expertise and support for ventricular assist devices (VADs). Assembles and operates pumps. Provides bedside monitoring, terminates VAD procedures and maintains records in accordance with local operating instruction. Add to Ancillary Procedures: Performs routine maintenance, schedules preventive maintenance and accurately maintains records of CPB equipment, auto transfusion apparatus, IABP units and VAD equipment. Monitors expendable supplies and equipment, submits orders as necessary to maintain adequate stock levels. Contractor's employees shall have the education, training and licensure, certification and experience necessary to provide services. The intended contract shall provide the Government the option to extend the contract in one-year increments for a base and four option years. Responses to this notice should include company name, address, point of contact, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) and must respond to the following questions: (1) Is your business a large or small business? (2) If small, does your firm qualify as a small, emergency business, or a small, disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and is your firm certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified "hub zone" firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned and Veteran-Owned? (7) Do your firm have an Environment Management System Certification (EMS)? Required Submittal: Contractors must submit the following: 1. A brief introduction into the company, summary of related capabilities information based upon the contract experience with Government or commercial customers listed below. 2. A summary of relevant contract experience, include a reference to the contract number, magnitude, place of performance, and points of contact for each example provided (name of the point of contact, address, e-mail, telephone number, fax number, and company's web page, if applicable). Submitted information shall be UNCLASSIFIED. 3. Any other information you think we need to evaluate your capabilities. Experience Documentation: The vendor shall provide a summary of experience, within the last 3 years, on performance and current customer operations for provided perfusion services. As a minimum, the summary shall include the following information for the projects proposed as relevant experience: 1) Customer Company Name, 2) Installation Location, 3) Date of Customer Acceptance, 4) Is the service still in use?, 5) Name of Customer Contact, 6) Telephone and Fax Number of Customer Contact, and 7) E-mail of Customer Contact. Any information submitted by respondents to this synopsis is strictly voluntary. The Government will not reimburse the respondents for any costs associated with their response. The Government reserves the right to contact additional customers to obtain information that will be used to evaluate the vendor's experience. This is not a request for proposals and in no way obligate the Government to award any contract. Responses to this notice shall be mailed to Bryan Armstrong, 350 Hangar Ave, Bldg 549, Travis AFB CA. Telephone and/or e-mail responses will not be accepted. Responses must be received in writing no later than 4:30 p.m. Pacific time,31 Aug 2009. Firms responding to this announcement which fail to provide all of the required information cannot be used to help the Government make the acquisition decisions which is the intent of this synopsis. THIS SOURCES SOUGHT IS FOR INFORMATION AND PLANNING PURPOSES ONLY AND IS NOT TO BE CONSTRUED AS A COMMITMENT BY THE GOVERNMENT TO A SET-ASIDE.
