- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0391 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-05. This acquisition is unrestricted under NAICS code 423450, size STD 100. NOTE: THIS IS AN UNFUNDED REQUIREMENT AND IS SUBJECT TO AVAILABILTY OF FUNDS. FUNDS AVAILABILITY: Funds are not available for this requirement, however, it is anticipated year-end funds may become available. Therefore, all quotes submitted in response to this RFQ must remain firm through 30 Sep 09. The requirements in this solicitation: Following equipment are required at General Leonardwood Army Community Hospital, Fort Leonardwood, MO: 1.RM284 Multi Station Sound Booth (8 Stations) QTY 1 2.Sound Room Installation QTY 1 3.Mounts (500267-05) QTY 2 4.BAS-200 Bio-Accoustic Simular - QTY 2 5.Manuals (2 Operator and 1 Service) QTY 1 Set MFG: Gordon N. Stowe Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to General Leonardwood Army Community Hospital Acceptance Location: General Leonard Army Community Hospital, 126 Missouri Ave, Ft Leonardwood, MO 65473. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation of Quotes: The Government will award a contract resulting from this solicitation to the responsible vendor whose quote, conforming to this solicitation, is Lowest Priced, Technically Acceptable. Award will be on an all or none basis. The following factors will be used to evaluate quotes: 1. Technical Acceptability: In order to be considered technically acceptable, vendors must demonstrate that all items proposed (if other than Brand Name) meet or exceed the salient characteristics listed above. 2. Price and 3. Delivery Schedule will be evaluated. The award decision will be based on the best on the best value to the Government. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation.
