Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number M00146-09-T-3336 is hereby issued as a Request for Quotation. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32 and Defense Acquisition Circular 98-108. This acquisition is 100% set-aside for small business. The North American Industry Classification System code 812990, with a small business size standard of $7 million. This requirement is for a firm fixed priced contract to provide Roman Catholic Ministry Program services in support of the mission and objectives of the Marine Corps Air Station Cherry Point chapel programs.(See attached Performance Work Statement) Performance of the following service (to begin before October 1, 2009 and continue for one year with the option for four additional years:Estimated dates are as follows:Line item 0001 - Base Period of Performance: 1 September 2009 - 31 August 2010 Line item 0002 - First Option Period of Performance: 1 September 2010 - 31 August 2011 Line item 0003 - Second Option Period of Performance: 1 September 2011 - 31 August 2012 Line item 0004 - Third Option Period of Performance: 1 September 2012 - 31 August 2013 Line item 0005 - Fourth Option Period of Performance: 1 September 2013 - 31 August 2014 The following provisions and addendum apply to this acquisition. FAR 52.212-1, Instructions to Offers - Commercial Items. FAR 52.212-1 Addendum. The following addition/changes are made to FAR Provision 52.212-1, which is incorporated by reference:The following provision(s) is/are hereby incorporated by reference:FAR 52.211-14 Notice of Priority Rating For National Defense Use, FAR 52.217-5 Evaluation of Options, FAR 52.233-2, Service of Protest, DFAR 252.212-7000, Offeror representations and certifications - Commercial Items. Evaluation procedures: 1. The Government will award a contract resulting from this solicitation to the offeror whose quote is the lowest price technical acceptable with a satisfactory record of past performance. 2. Technical acceptability will be evaluated based upon the extent to which the offeror provides evidence of or concisely and accurately describes addresses the following: a. Ordained Catholic Priest in good standing and in possession of an Endorsement by the Archdiocese for the Military Services (provide a copy). b. Possess a Master of Divinity Degree (provide a copy). c. Be granted Faculties by the Archdiocese for the Military Services to includedelegation for the administering of Catholic Rites/Services to military service members (provide a copy). d. Provide a Certificate of Ecclesiastical Endorsement (provide a copy). e. Possess general working knowledge of Microsoft Word, Microsoft Excel, andMicrosoft Outlook or equivalent software and desktop computer hardware (describe experience with this software). f. Possess an understanding of military customs, courtesies, traditions, protocol, and honors rendered in conjunction with military ceremonies (describe how this experience has been obtained). g. Must demonstrate ability to interact with people from all socio-economic backgrounds, civilian and military (including family members and retirees) (describe how this experience has been obtained). h. Must demonstrate clear, concise, and professional oral and written communication skills through past parish/worship experience (describe how these skills have been obtained). FAR 52.212-3, Offer Representation and Certifications-Commercial Items, Alternate I. A completed copy of this provision shall be submitted with the offer. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
