- SF1449 to be completed and submitted with quote RFQ - HU0001-09-T-0346 The Uniformed Services University of the Health Sciences (USU), located on the National Naval Medical Campus in Bethesda, MD, has a requirement for the purchase of Laboratory Equipment (Sterile Media-Prep Machine and Plate Filling Machine). This is a combined synopsis/solicitation for commercial items prepared under the authority of FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This is a request for quotations (RFQ) in accordance with FAR Part 13 - Simplified Acquisition Procedures. It is the responsibility of the contractor to monitor FedBizOpps for any amendments to the solicitation. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular 05-35. This procurement is set aside for small businesses only. For this procurement, the North American Industry Classification System (NAICS) code is 423490 (Other Professional Equipment and Supplies) with a size standard of 100 employees. EVALUATION FACTORS The government will award a Firm Fixed Price (FFP) contract resulting from this solicitation to the responsible offeror whose quote, conforming to this announcement, will be most advantageous to the government. Award will be made on a best value determination considering price and non-price factors in accordance with FAR 52.212-2 - Evaluation - Commercial Items Clause. •1. Technical - Offerors are to provide product literature demonstrating the products quoted meets the specifications and provide warranty information. •2. Price (to include delivery) - Offerors are to provide the unit of issue, the unit cost and the total price for the aforementioned estimated quantities The contractor shall provide pricing as specified in the attached solicitation (1449). Further instructions concerning the specifications of the laboratory equipment is found in the attached statement of work. Interested offerors must submit the completed SF 1449 and product literature as part of their quote package. Additionally, offerors must complete 52.212-3 ( Instructions to Offerors-Commercial Items) and 252.225-7035 ( Buy American Act-Free Trade Agreements-Balance of Payments Program Certificate) contained in the SF 1449. Interested offerors must also provide DUNS number and tax identification numbers. To be eligible for contract award, a contractor must be registered with the Central Contractor Registration (CCR) database. For instructions on registering with the CCR, please see the CCR website at www.ccr.gov. USUHS reserves the right to award without discussions. All questions must be directed by email to ggermana@usuhs.mil and timothy.bunting@usuhs.mil (as secondary contact). Questions must be received by 12:00p.m. (EST) on 10 August 2009. Responses to questions will be addressed as an amendment to the RFQ. No questions will be answered over the phone. Quotes shall be accepted by email only.
