Description
This Performance Work Statement (PWS) covers the requirements of the United States Army Medical Research and Material Command's (USAMRMCs) Telemedicine and Advanced Technology Research Center (TATRC) for research consultation, support and analysis for a TATRC based AAMTI project to be conduct at Walter Reed Army Medical Center (WRAMC) entitled A pilot study of store and forward telepsychiatry in military settings 2. Background/Purpose As part of TATRCs AMEDD Advanced Medical Technology Initiative (AAMTI) a demonstration trial entitled A pilot study of store and forward telepsychiatry in military settings is occurring in partnership with WRAMC. A significant percentage of returning Operation Iraqi Freedom and Operation Enduring Freedom service members have mental health care needs including posttraumatic stress disorder, depression and substance abuse increasing the burden of the Department of Defenses system of mental health care. A new model of remote care, store and forward telepsychiatry, holds promise to address work load burden on the current psychiatric work force as well as improve the quality of available psychiatric care. In store and forward telepsychiatry initial psychiatric assessments are conducted either by a nurse practitioner or social worker and recorded using a high quality digital video camera. The interview is tagged for clinically relevant material by the clinician using a clicker when they deem clinically relevant material is being discussed so that a later psychiatrist reviewer can know where in the interview to select sections to observe. A summary of the patients history along with the recorded video interview is forwarded electronically to a psychiatrist who then reviews the material and provides an assessment and psychiatric treatment plan. This plan is transmitted to the patients primary care provider. The primary provider then manages the patients psychiatric conditions using the psychiatric consultation as well as email and phone follow-up consultation with the psychiatrist. This demonstration trial will test this new modality of care using a randomized controlled design with 120 patients seeking psychiatric care in the Walter Reed Medical Center Tele-Behavioral Health Service. Outcomes measures will include measures of clinical symptoms, systems of care measurements and comparison of diagnoses given during store and forward assessments. If this system proves feasible and reliable in its adaptation to the Department of Defense it could have significant benefits in providing remote care and consultation, amplifying the current psychiatric work force, addressing work load issues and improving the quantity and quality of psychiatric consultation available throughout the system. In order to carry-out this project consultation services are needed to appropriately design and adapt store and forward telepsychiatry for WRAMC. Additional services are required for evaluation of this trial from those with expertise in store and forward telepsychiatry for project evaluation, statistical analysis and consultation and manuscript/report production. 3. Objective Vendor shall advise and aid in the design and adaptation of store and forward telepsychiatry for WRAMC for the demonstration pilot. The vendor shall also help to refine the planned evaluation of the pilot including data collection methods and sources. Once data has been collected the vendor shall work with the Primary Investigator (PI) for the project (Dr. Jay Shore) to aid in the sorting and cleaning of the data, conduct the primary statistical analysis and evaluation of the data and work to create reports including submission of manuscripts for publication on the outcomes of the trial.
