Description
Laser Safety Officer Sources Sought Synopsis: The Department of Veterans Affairs is conducting a sources sought for a requirement to provide monitoring and enforcing control on laser hazards, and to effect the knowledgeable evaluation and control of laser hazards at the VA North Texas Health Care System (VANTHCS) campus, 4500 S. Lancaster Road, Dallas, TX 75216. Service provider must have been engaged in evaluating laser hazards and installing and establishing laser control to ensure compliance with requirements of medical surveillance of the laser found in American National Standards Institute (ANSI) Z136.3-2005, American National Standard for Safe Use of Lasers in Health Care Facilities. The service provider will oversee the VA North Texas Health Care Center Laser Safety Program. A comprehensive safety review will be conducted for every Class 2 or higher laser system where direct beam viewing cannot be avoided. A Nominal Hazard Zone (NHZ) will be established in the space around a Class 3a or higher laser operated in open areas within the hospital in which exposure to laser light can exceed the Maximum Permissible Exposure (MPE) limit. Initial safety evaluations will be performed and reviews of laser-related Integrated Work Sheet/Safety Plans (IWS/SP) will be completed. Technical approval will be provided for personal protective equipment used to include laser safety eyewear for each laser. Annual self-assessment of the Laser Safety Program will be conducted and a report on any deficiencies will be provided to the Senior Industrial Hygienist no later than September 30th of each year. The service provider will be responsible for the evaluation and approval of modifications to lasers and lasers being moved offsite and will provide VA staff education for nurses, surgeons and technicians, including all new employees in the use of the laser.
