- THIS IS NOT A NOTICE OF RFP ISSUANCE. THIS EFFORT IS CONSIDERED COMMERCIAL IN NATURE AND ANY RESULTANT CONTRACT WILL BE AWARDED IAW FAR PART 12. Patrick Air Force Base, Florida anticipates competitive acquisition(s) to provide and place the following professional medical staff and medical clerical staff to commence work on 01 October 2009 for a period of twelve (12) months. Professional Medical Staff: - Full time Radiologist (shall have graduated from an approved school and completed a residency or fellowship acceptable to the US Air Force Surgeon General, shall possess a valid unrestricted license to practice diagnostic imaging as a radiologist from the State of Florida, be board eligible or board certified by ACR and have provided a minimum of five (5) years of service and a minimum of six (6) months with the past twelve (12) months), one each - Part time Radiologist (shall have graduated from an approved school and completed a residency or fellowship acceptable to the US Air Force Surgeon General, shall possess a valid unrestricted license to practice diagnostic imaging as a radiologist from a U.S. state, shall be board eligible or board certified by ACR and shall have provided five (5) years services a minimum of six (6) months within the past twelve (12) months), one each - Part time Social Worker (shall possess a Masters of social Work degree or be a Clinical Psychologist and must have graduated from an approved school and completed a residency or fellowship acceptable to the Air Force Surgeon General, shall possess a valid unrestricted license to practice social work or psychology from the State of Florida, shall be board eligible or board certified and shall have provided psychotherapy services a minimum of twelve (12) months in the last three (3) years as a therapist in the outpatient mental health or primary care setting), one each - Full time Registered Nurse for the Family Practice Clinic (shall have at least twenty four (24) months of experience within the last seven (7) years. This experience shall have been in a primary care clinic setting), two each - Full time Physical Therapy Assistant (shall have graduated from a physical therapy assistant program accredited by the American Physical Therapy Association (APTA) and have at least twelve (12) months of experience within the last three (3) years), one each The following medical clerical staff shall perform according to the National Patient Safety Goals, the Health Insurance Portability & Accountability Act (HIPAA) and provide administrative skills within the scope of their professional certification as requested by the US Air Force: Full time Diagnostic and Therapeutic Administrative Clerk, four each Full time Medical Coding Auditor, two each Full time Medical Records Technician, four each Full time Referral Management Center Clerk, one each Full time Medical Administrative Office Clerk, one each Only HubZone, 8(a) status or otherwise classified as a small disadvantaged veteran owned business sources capable of providing these services are being sought and should address their status in their responses. In addition to the information provided below entitled "Capabilities Package," responding parties must also indicate their size status in relation to the applicable Standard Industrial Classification (SIC) Code 8011 (size standard $10.0M) and North American Industry Classification System (NAICS) code 621111. Capabilities Package. All interested firms should submit a capabilities package that outlines the firm's capabilities in providing the required services. Contractor is required to provide all management and labor required for the operation and management of the required services. The capabilities package should be brief and concise, yet clearly demonstrate an ability to meet the stated requirements. Recent, relevant experience in all areas should be provided.
